Atypical Nummular Headache or Circumscribed Migraine: The Utility of Pressure Algometry

Abstract: Nummular headache is a type of headache that can be identified by continuous or intermittent pain occuring in a well-circumscribed area. The symptoms of nummular headache can overlap with those of migraine headache, and a failure to reach an accurate diagnosis can affect the choice of treatment. The authors report a case involving a 21-year-old woman with headache symptoms that appeared to represent nummular headache, for whom treatment with the typical first-line agent was unsuccessful. The authors subsequent… Show more

“… 11 In the case described, the effectiveness of PEA as a prophylactic therapy for NH is clear, as demonstrated by the improvement in rates of pain measuring scales, with excellent clinical response, as evidenced also by the gradual improvement in pain symptoms, despite the reduction of topiramate. 12 No adverse effects related to the drugs administered were reported during the observational periods, but a case series or clinical trial is necessary for the future.…”

“…It is characterised by superficial pain, sharply contoured, with a fixed round or elliptical shape and size, which can range from 1–6 cm (2–3). Its pathophysiology suggested impairment of superficial sensory terminations, considering its superficial topography, precise location, and the frequent presence of circumscribed allodynia and trophic changes as a marker of superficial sensory fiber dysfunction (47). Preventive treatment is required in two third of patients.…”

“…NH is a continuous or intermittent pain which is commonly oppressive head pain circumscribed within a rounded or elliptical area, typically 1–6 cm in diameter (Pareja et al 2002 ; IHS 2013 ). But, NH may present atypical features resembling a migraine pattern such as paroxymal episodes, accompaniments of nausea, photophobia and phonophobia (Dai et al 2013 ), triggering or aggravating factor of physical exercise (Mulero et al 2013 ; Baron et al 2015 ), or relation to menstruation (Robbins and Grosberg 2010 ). Our patient also showed pain features resembling a migraine pattern of episodic pain accompanied with dizziness, head heaviness, photophobia and phonophobia, triggered by anxiety or dysphoria.…”
