Avoided Hospitalization Criteria: Validating the Impact of a Community-Based Palliative Care Program

Abstract: Objective: This report describes the experiences of a community-based palliative care (CBPC) program's efforts to understand the patterns of hospital utilization, specifically utilization reduction experienced by admitted patients.
