Multidetector CT Angiography Versus Digital Subtraction Angiography for Aortoiliac Length Measurements Prior to Endovascular AAA Repair

Abstract: CTA produces better intra and interobserver correlations in measuring vessel length than DSA. It has the potential to replace DSA as an imaging method before EVAR.

“…The largest study of 52 patients showed a sensitivity of CTA for abdominal arterial stenosis of 82%, with a specificity of 100% [17]. Furthermore, CTA was shown to have better intraobserver and interobserver variability in comparison with DSA [18]. Interobserver variability also exists among duplex ultrasound measurements of gastrointestinal arterial flow parameters [19,20].…”

“…Thus, our data are likely valuable in this clinical setting. Furthermore, with the advent of MDCT, catheter-based angiography has been replaced by CTA as the primary diagnostic procedure before the abdominal aortic endovascular stent graft deployment (Diehm et al, 2004). Accurate definition of the patient's renal vascular anatomy plays an important role in achieving satisfactory aortic endovascular graft placement (Broeders and Blankensteijn, 2000).…”

“…DSA also does not show true aneurysm size if there is mural thrombus. DSA is still an important tool in the endovascular treatment of the AAA (6).…”
