B‐scope Echoencephalography in the Infant

Abstract

SUMMARY The difficulty of diagnosing and investigating the infant with suspected intracranial pathology is examined and the advantages of ultrasound are discussed. A comparison of A and B‐mode techniques suggests that B‐mode echoencephalography is necessary to identify intracranial anatomy. Three types of B‐mode system are considered, and after examination of two groups of patients by a modified compound scanning system or flexible‐faced water‐bath, it is concluded that either technique will allow identification of gross intracranial anatomy. The type of presentation in the 63 infants examined determined whether B‐mode echoencephalography should be the only investigation, or merely the initial one. When presentation and B‐scan findings were considered together, the rôle of B‐scope echoencephalography in any particular infant becomes clearer. RÉSUMÉ Echoencéphalographie‐B chez le nourrisson Les auteurs examinent les difficultés du diagnostic et de la recherche chez le nourrisson suspect de pathologie intra‐crânienne et discutent les avantages des ultra‐sons. Une comparaison entre les techniques A et B suggère que l'échoencéphalographie‐B est nécessaire pour identifier l'anatomie intra‐crânienne. Trois types de B sont envisagés et après un examen de deux groupes de nourrissons par un système de balayage modifié ou par contact hydrique à face flexible, on conclue que l'une ou l'autre technique permet une identification grossière de l'anatomie intra‐crânienne. Le type de présentation chez les 63 nourrissons examinés a déterminé si l'échoencéphalographie‐B devait être la seule investigation ou seulement la première. Quand on tient compte à la fois de la présentation et des données du balayage B, le rôle de l'echoencéphalographie‐B chez chaque enfant devient plus clair. ZUSAMMENFASSUNG B‐Bild Echoencephalographie beim Säugling Die Schwierigkeit der Diagnostik und Untersuchung beim Säugling, bei dem der Verdacht auf einen intracraniellen Prozeß besteht, wird dargestellt und es werden die Vorteile der Ultraschalldiagnostik erörtert. Ein Vergleich zwischen A‐ und B‐Methode läßt erkennen, daß eine B‐Bild Echoencephalographie erforderlich ist, um die intracraniellen anatomischen Strukturen zu identifizieren. Drei Typen der B‐Methode werden erprobt und nach der Untersuchung von zwei Säuglingsgruppen mit einem modifizierten Aufzeichnungssystem oder einer Wasserbadtechnik kommt man zu dem Schluß, daß beide Techniken geeignet sind, gröbere intracranielle Strukturen zu erkennen. Das klinische Bild der 63 untersuchten Säuglinge entschied ob das B‐Bild Echoencephalogramm als einzige oder erste Untersuchungsmethode angewendet werden sollte. Wenn man das klinische Bild und den B‐Scan zusammen betrachtete, wurde die Rolle der B‐Bild Echoencephalographie bei jedem einzelnen Kind klarer.
