Among the 34 publications (32 RCTs) included in this review, (Miranda et al, 2015) and (Rand et al, 2015) were earlier publications reporting on the same RCTs as (Miranda et al, 2019) and (Rand et al, 2017), and will henceforth be referred by the latest publications.
