Introduction Rectovaginal fistula (RVF) is an abnormal communication between the vagina and the rectum. RVFs caused by Bartholin's gland infection are very rare. We present the case of recurrent rectovaginal fistula complicating a bartholin's gland abcess successfully treated with a Martius flap. The aim of this work is to demonstrate the possibility of complication of bartholin's gland infection by a rectovaginal fistula and the efficacy of Martius flap procedure for recurrent RVF of the low third part of the vagina. Observation It is a 30-year-old woman admitted to our department for recurrent RVF due to an abscess of the Bartholin's glands. She was previously treated with a vaginal advancement flap which failed with persistence of the fistula and its symptoms. The patient underwent a RVF repair by Martius flap with complete healing of the fistula. Discussion Rectovaginal fistula is a complex pathology with psycho-social, individual, family, religious and ethno-environmental repercussions. Its main aetiologies are obstetric, rectal surgery. Several techniques including the vaginal or anal advancement flap and the Martius flap are used for the treatment of rectovaginal fistulas. For recurrent fistulas, the Martius flap seems to be the most indicated with better results. Conclusion Rectovaginal fistula remain a challenge for surgeons and have major psycho-socio-economic repercussions for the patient. The complication of Bartholin's gland infection by rectovaginal fistula is rare. The Martius flap technique is the method of choice for recurrent rectovaginal fistulas of the lower third of the vagina or in association with other pathologies.

The review is devoted to the most pressing issues of etiology, classification, diagnosis and treatment of rectovaginal fistulas. The psychosocial impact of a rectovaginal fistula on women’s quality of life cannot be overemphasized. The most common causes of rectovaginal fistula are postoperative and obstetric complications, inflammatory bowel disease, complications of radiation therapy, and neoplasms. A combination of advanced imaging techniques, physical examination, and clinical experience can help determine the location and cause of a fistula. The therapeutic options are varied, but the results are often unsatisfactory. In this regard, the choice of a strategy for the treatment of rectovaginal fistulas should be based on etiology, localization and concomitant pathology. Despite the prevalence and serious consequences of rectovaginal fistulas, there is no generally accepted scientifically based approach to surgical treatment. Collaboration between obstetricians-gynecologists and colorectal surgeons, detailed preoperative assessment, consideration of optimal treatment options and correct surgical technique can achieve a successful outcome in most patients. The aim of the study is to evaluate, on the basis of literature sources, the problem of diagnosis and treatment of patients with rectovaginal fistulas and modern possibilities for improving the results of surgical interventions in this pathology. The methodological basis of the analysis was the study of domestic and foreign literature devoted to this problem. Scientific articles were selected for the period 2016–2021, with the largest randomized controlled trials in this area. The article provides an overview of the main management strategies and technical approaches that are currently effective in eliminating this socially disabling condition.

Pelvik organlar ile perineal yüzeyler arasında birçok mekanizmayla birbirinden farklı apse formasyonları gelişebilmektedir. Anorektal apseler bu hastalıkların en yaygın sebeplerindendir. Klinik olarak daha çok lokal bir enflamasyon şeklinde karşımıza çıksa da, özellikle altta yatan başka patolojiler olduğunda komplikasyonlar ile birlikte seyredebilir. Olguların çoğunda tedavi, drenaj ve antibiyotik tedavisidir. Ancak klinik tabloların anatomik ve terminolojik açıdan sınıflandırılması, tedavi yönetimi ve komplikasyonların takibi açısından çok önemlidir. Bu yazıda, akut batın ve sepsis kliniği ile başvuran periüretral, perivajinal, perirektal boşlukları doldurarak gluteoperineal sahaya yayılan ve vajen lümenine spontan drene olmuş bir anorektal apse olgusu sunuldu.
