Basic theories and concepts of chest wall surgery

Pectus excavatum is a common thoracic deformity, and Nuss procedure is considered as the standard operation for the treatment of this deformity. However, there are often cases of Nuss procedure failure in the clinic. Once such a situation occurs, it often needs to be operated again. Since Nuss procedure can cause adhesions behind the sternum and in the bilateral thoracic cavities, if Nuss procedure is still used in the reoperation, it is not only very difficult, but also possible to cause heart and lung injuries. Therefore, Nuss procedure is not an ideal choice for reoperation. Wang procedure is a new method to treat the deformity of depressed chest wall. Since this operation is mainly performed outside the chest wall and rarely involves the structures in thoracic cavity, it is suitable for reoperation after the failure of Nuss procedure. This article reports a 12-year-old boy who had a failed Nuss procedure. We used Wang procedure for treatment and achieved satisfactory results.

Wang technique: A simple and practical steel bar fixation technique in thoracic deformity surgery

Minimally invasive thoracic deformity surgery often requires the use of steel bars. After these bars are placed in the chest wall, they need to be firmly fixed. In the past, most of the fixation techniques needed special fixation plates. Although these methods have certain advantages, their disadvantages are more impressive. We have invented a technique specially used for steel bar fixation. This technique is simple, practical and easy to operate. We used it routinely in all kinds of thoracic deformity operations and achieved satisfactory results. In the past few years, we have carried out thoracic deformity surgeries in more than 300 hospitals in China, and this technique is adopted in all our operation. Because of the obvious advantages, it is unanimously recognized by the peers. It has become a popular technique in chest wall surgery circle. In order to better standardize the naming of this technique, we named it Wang technique. In this paper, the specific contents of this technique are introduced in detail through a case of Wung procedure on pectus excavatum.

Wenlin procedure: Minimally invasive surgery for barrel chest

Wenlin procedure is a special operation designed by us. In the early stage, it was specially used for the treatment of pectus carinatum. Later, we found that it also had an effect on other protrusion deformities. Barrel chest can be regarded as the overall protrusion of chest wall, so it is also a kind of protrusion deformity. Because there is protrusion, Wenlin procedure should also play a role. We used this kind of operation in the operation of barrel chest and obtained satisfactory results. Our experience shows that Wenlin procedure is an ideal choice for the treatment of barrel chest.
