Behaviour‐based pain scales: Validity and interrater reliability of BPS‐NI and PAINAD‐G on general wards

Abstract: Background: Measuring pain intensity in patients unable to communicate is a challenge on general wards. Observation-based pain scoring tools have been used for patients with dementia or in critically ill intensive care unit (ICU) patients. However, there is no established or validated assessment tool for non-ICU patients without dementia related cognitive deficiencies who cannot communicate. The "Behavioural Pain Scale Non-Intubated" (BPS-NI) and the "Pain Assessment In Advanced Dementia-German" (PAINAD-G) are p… Show more

“…We appreciate the constructive discussion of our recent publication (Sabour, 2023;Wandrey et al, 2022). In response to the points raised regarding our statistical methods, we agree that alternative models are available to evaluate score validity and interrater reliability.…”

“…In response to the points raised regarding our statistical methods, we agree that alternative models are available to evaluate score validity and interrater reliability. In our study, we calculated an interrater reliability of BPS-NI and PAINAD-G using Cohen's Kappa resulting in substantial congruence for BPS-NI (Cohens-Kappa 0.71) and moderate congruence (Cohens-Kappa 0.48) for PAINAD-G, respectively (Cohen, 1992;McHugh, 2012;Wandrey et al, 2022). As proposed, we analysed data with an additional Bland-Altman plot (Martin Bland & Altman, 1986) (Figure 1a,b).…”

“…In particular, dementia patients were the most frequently excluded (154 patients). Patients with dementia have been reported to give unreliable self‐reports of pain intensity, and validity may be ensured by utilizing behavior‐based pain scales (Wandrey et al., 2023). Future studies may wish to consider study designs that allow for the inclusion of patients with dementia.…”
