Best practice approach for redo-surgeries after sleeve gastrectomy, an expert's modified Delphi consensus

Abstract: Background Sleeve gastrectomy (SG) is the most common metabolic and bariatric surgical (MBS) procedure worldwide. Despite the desired effect of SG on weight loss and remission of obesity-associated medical problems, there are some concerns regarding the need to do revisional/conversional surgeries after SG. This study aims to make an algorithmic clinical approach based on an expert-modified Delphi consensus regarding redo-surgeries after SG, to give bariatric and metabolic surgeons a guideline that might help … Show more

“…Moreover, 9/11 patients with GERD improved, which may be promising for revisional procedure for GERD. However, the latest Delphi consensus does not recommend SASI as a revisional procedure after SG for GERD symptoms [ 14 ]. Due to the small number of people analysed, further work is needed to assess the efficacy of SASI as a revisional surgery.…”

“…Musella et al [22] reported a multi-central retrospective anaylsis of patients undergoing OAGB after failed restrictive procedures and showed a 74.5% excess BMI loss at 20.8 months follow-up with 8.6% overall complication rate. In an expert's modified Delphi consensus on the best approach for redo surgeries after SG, OAGB was agreed by 84.7% of the committee to be an acceptable procedure after SG [23].…”

“…44 The most recent expert-modified Delphi consensus on revisions after SG recommended an algorithmic approach with medical therapy first prior to considering RYGB. 45 Staple line leak and bleeding can be devastating complications after SG, and several adjuncts have been proposed to reduce this risk. These include staple-line reinforcement with buttress material, oversewing of the staple lines, and application of fibrin glue.…”
