Bilateral sciatic neuropathy with severe rhabdomyolysis following venlafaxine overdose

Abstract: Rationale:Venlafaxine is an antidepressant and anxiolytic agent that functions by inhibiting central serotonin and norepinephrine reuptake, and it is a relatively recently introduced drug. In particular, overdose of venlafaxine has been reported to cause severe cardiac toxicity including ventricular tachycardia, prolongation of QT interval, and seizure or severe muscular injury. However, reports describing venlafaxine-induced rhabdomyolysis with neuropathy remain scarce. Accordingly, we report such a case invo… Show more

“…Venlafaxine was administered orally in all cases. Of the 12 patients, 5 patients received venlafaxine at doses of ≤225 mg/d (cases 1-5), whereas the remaining 7 patients received doses exceeding 225 mg/d (cases [6][7][8][9][10][11][12]. Seven patients received venlafaxine as monotherapy, 4 patients received it in combination with another drug, and 1 patient received it in combination with 3 other drugs.…”

“…The treatment of sciatic neuropathy is generally medication specific to the pain, such as tricyclics, anticonvulsants, and topical analgesia. [3] In some cases, a simple combination of anti-inflammatory medication and mild exercise may also be prescribed. However, treatment of sciatic neuropathy solely with medication often leads to unsatisfactory outcome.…”

“…According to various authors, venlafaxine should be recommended for use in primary care as a second-line option following the treatment with selective serotonin reuptake inhibitors (Thase et al, 2017;Schueler et al, 2011). Similar to other antidepressants, venlafaxine has manifested a number of moderate collateral symptoms such as fatigue, nausea, somnolence, or mild gastrointestinal problems; however, eventually, the drug can lead to more serious problems, which include severe lactic acidosis, interstitial pneumonitis, neuropathy, different forms of adult and prenatal cardiomyopathy, as well as liver and kidney affections (Paulis et al, 2018;Ko et al, 2018).…”
