The treatment of comminuted tibial shaft fractures in canine patients is burdened by significant risk which involves bone healing complications, such as delayed bone union. Complications may result from iatrogenic damage to blood vessels during fracture stabilization. To minimize this risk, treatment methods increasingly often rely on the concept of biological osteosynthesis. One of such methods involves the treatment of fractures with the use of new hybrid fixator consisted of an interlocking nail connected with type I external fixator. Connection of the nail with external fixator has been recently developed to maximize treatment efficiency. This manner of stabilization increases bone-fixator construct strength on forces acting in the place of fracture. It also enables fracture fixation with minimal damage of the blood supply of bone fragments. This article describes surgical procedure of stabilization of comminuted tibial bone fractures in four dogs by the use of interlocking nail connected with external fixator type I, discusses and evaluates the results of clinical treatment with the involvement of the said fixator. To control bone consolidation process the radiograms were taken in 6 and 8 week of healing.In all cases, the reviewed methods of clinical treatment were successful in producing bone union after eight week of healing. During the whole period of observations no complication was observed. In all cases the intramedullary nail were left in the medullary canal after the healing process was finished. The fixator supported quick restoration of limb function after treatment.

As hastes bloqueadas além de permitirem estabilização rígida de fraturas, possuem vantagens biomecânicas quando comparadas a outras técnicas de imobilização, por atuar ao longo do eixo mecânico central do osso, além de preservar os conceitos de padrões biológicos de osteossíntese . O uso limitado de hastes bloqueadas em gatos se atribui ao fato da cavidade intramedular ser pequena, limitando o tamanho e diâmetro dos pinos. Relatam-se casos de 10 felinos que apresentavam fraturas fechadas simples ou múltiplas da diáfise femoral. Os animais pesavam entre 3,5 e 5 kg, o que permitiu a utilização de hastes de 4,0mm e 4,7mm de diâmetro. Para inserção da haste, adotou-se a via retrógrada ou a normógrada e foram realizados bloqueios com quatro parafusos de titânio de 2,0 mm de diâmetro. Complicações intra-operatórias não ocorreram, entretanto um animal sofreu fratura de colo femoral e fratura transversa distal ao implante, três dias após o ato cirúrgico, devido a novo trauma. Os animais foram submetidos a exames radiográficos até quatro meses após a intervenção, evidenciando-se formação de calo ósseo secundário e consolidação óssea em período de 61-89 dias. Clinicamente, ocorreu esporadicamente alteração na marcha por leve claudicação de apoio definida por escala de claudicação, entretanto os animais iniciaram o apoio adequado entre três e oito dias após o ato operatório. Complicações pósoperatórias ocorreram referentes ao animal que sofreu novo trauma, entretanto notou-se consolidação do foco primário tratado pelo implante com 61 dias e consolidação da nova fratura num período total de 150 dias do início do tratamento conservativo por penso esparadrapado e tala. Conclui-se que o uso de haste bloqueada em felinos foi adequado uma vez que houve consolidação óssea em todos os casos, com retorno precoce da função do membro, permitindo a deambulação.

Zusammenfassung: Die Fortschritte in der Osteosynthese beim Pferd, die während den letzten 50 Jahren erzielt wurden, werden in verschiedenen Kapiteln erörtert und aufgeteilt in Schrauben-und Plattenfixationen. Bei den Letztern wird die Entwicklung der verschiedenen Systeme kurz beschrieben und deren Vor-bzw. Nachteile aufgelistet. Die Verriegelungsplatten und Schrauben sind heute die Produkte der Wahl bei der Plattenosteosynthese und es lohnt sich, die höheren Kosten zu investieren, denn die Erfolge mit diesen Implantaten sind deutlich besser. Dabei ist es nicht nötig, nur Verriegelungsschrauben zu verwenden, denn zwei Schrauben auf jeder Seite der Fraktur genügen in den meisten Fällen, um die Stabilität zu erreichen. Die restlichen Plattenlöcher können mit Kortexschrauben versehen werden, die deutlich preiswerter sind. Es wird auch auf den Einsatz der Externen Fixation, Transfixation und der Verriegelungsnägel kurz eingegangen. Die Prä-vention von Infektionen bei der Osteosynthese, die Computer-assistierte Chirurgie, der intraoperative Einsatz von Computertomographen, die minimalinvasive Frakturbehandlung, sowie die Frakturbehandlung am stehenden Pferd werden kurz besprochen. Ein kurzer Ausblick auf künftige Entwicklungen bildet den Abschluss.Schlüsselwörter: Fortschritte / Osteosynthese / Frakturen / Pferd / LCP / Orthopädie / Traumatologie Advances in osteosynthesis in the horseThe advances in osteosynthesis in horses that have been achieved during the last 50 years will be discussed in different chapters and divided into screw and plate fixations. In the latter the various systems, as they were developed are briefly described and their advantages and disadvantages listed. The locking plates and screws are now the implants of choice in the plate fixation, and it is worth to invest in the higher costs because these implants, if applied correctly, result in a better success rate. It is not necessary to use exclusively locking head screws along the plate; two screws on each side of the fracture are in most cases sufficient to achieve superior stability. The remaining plate holes can be filled with cortex screws, which are significantly more economical. The possibilities of fracture management with the help of external fixation, transfixation pins and the locking nails are briefly discussed. Prevention of infections in osteosynthesis, the use of computer-assisted surgery, the intraoperative use of computed tomography, minimally invasive osteosynthesis, and fracture management in the standing horse are mentioned. A brief outlook on future developments concludes this essay.
