Blood flow modification might prevent secondary rupture of multiple pancreaticoduodenal artery arcade aneurysms associated with celiac axis stenosis

Abstract: A pancreaticoduodenal artery arcade aneurysm (PDAA) is rare and often associated with celiac axis stenosis by the median arcuate ligament. Although rupture risk of the PDAA is not related to its size, treatment guidelines are absent. Here we describe a 59-year-old woman with multiple ruptured PDAAs associated with celiac axis stenosis who was successfully treated with coil embolization. As follow-up computed tomography revealed rapid expansion of residual PDAAs and new gastric artery dissection, median arcuate… Show more

“…5,6 In the published literature, celiac trunk stenosis accounts for approximately 63% of cases involving true PDA aneurysm formation. 1,2,7 A literature review by Vandy et al showed the main symptom associated with PDAA is abdominal pain observed in 54% of cases, with up to 26% being asymptomatic and the remainder with vague other symptoms. 8 Many PDAAs are found incidentally on CT scans for evaluation of unrelated pain or for followup of other disease processes.…”

“…15,16,9 Some advocate revascularization if celiac artery occlusive disease exists and if PDAA treatment would compromise collateral blood supply to the liver to avoid catastrophic hepatic ischemia. 17,7,18 An endovascular approach is our preferred management for both elective and ruptured cases. An endovascular approach is usually more expeditious than open surgery and associated with diminished blood loss and faster recovery.…”
