BMSC-Derived Exosomes Alleviate Intervertebral Disc Degeneration by Modulating AKT/mTOR-Mediated Autophagy of Nucleus Pulposus Cells

Abstract: The pathogenesis of intervertebral disc degeneration (IDD) is still unclear. It has been shown that the pathological process of IDD is most closely related to inflammation of nucleus pulposus cells (NPCs), in which inflammatory factors play an important role. Exosomes are the main paracrine mediators and are microvesicles with biological functions similar to those of the cells from which they are derived. Studies have shown that bone mesenchymal stem cells (BMSCs) can inhibit apoptosis of NPCs by sending exoso… Show more

“…However, RCD has a precise molecular mechanism that can be regulated at the genetic and pharmacological levels [ 115 ]. MSC-EXO-derived ncRNAs are associated with multiple RCD modalities, such as apoptosis, ferroptosis, pyroptosis and autophagy [ 116 – 119 ].…”

“… Zhu et al (2020) further demonstrated that MSC-derived exosomes reduced the IL-1β-induced secretion of multiple inflammatory factors as well as achieving the targeted inhibition of the MAPK signaling pathway by packaging mircoRNA-142-3P . Bone-MSC-derived exosomes were also reported to inhibit the expression of IL-1β and TNF-α and promote autophagy ( Xiao et al, 2022 ). Luo et al (2021) compared the effects of exosomes derived from normal and degenerative CEP stem cells on IDD and found that exosomes derived from normal CEP stem cells had a better effect in terms of promoting autophagy ( Figure 3 ).…”

“…Similarly, under the stimulation of external effects, miRNA-21 can be transfected into mesenchymal cell exosomes, enhancing the ability of MSCs to differentiate into osteoblasts and promote vascular regeneration[ 10 ]. In other related studies, exosomes derived from bone marrow MSCs inhibited apoptosis and inflammation by upregulating autophagy through the protein kinase β/mammalian target of rapamycin signaling pathway[ 11 ]. MSCs-derived exosomes can also inhibit apoptosis of nucleus pulposus cells through miRNA-532-5p transport, demonstrating that this transport is also effective in IDD therapy[ 12 ].…”
