Bone marrow biopsy in the initial staging of Hodgkin's disease

Abstract: Results of bone marrow biopsy were retrospectively evaluated in 120 previously untreated patients with Hodgkin's disease. The incidence of bone marrow involvement was 13%. All patients with marrow invasion had B symptoms and/or clinically advanced disease. When patients with bone marrow involvement were compared to those without there were significant differences in the incidence of B symptoms, the clinical stage, hemoglobin levels, leukocyte counts, platelet counts, and serum levels of lactate dehydrogenase a… Show more

“…Examination of the patients treated at our institution over the past 27 years demonstrated that bone marrow involvement is rare in paediatric patients with HL. The incidence of bone marrow involvement was 6AE5% (4/62), and was consistent with adult studies that demonstrated an incidence of 4AE8-18% (Menon & Buchanan, 1979;Doll et al, 1989;Ellis et al, 1989;Munker et al, 1995;Howell et al, 2002;Vassilakopoulos et al, 2005). Our incidence was higher than in the paediatric study reported by Mahoney et al (1AE8%), which may be attributed to the fact that our institution routinely performs bilateral BME, which was not performed consistently in their report.…”

“…The most consistently reported risk factors for bone marrow involvement in adult HL include advanced age, the presence of ‘B’ symptoms, anaemia, leucopenia, thrombocytopenia and advanced clinical stage (Doll et al , 1989; Ellis et al , 1989; Munker et al , 1995; Spector et al , 1997; Howell et al , 2002; Vassilakopoulos et al , 2005). Those respondents to our survey, who did not perform routine BME, were influenced to do so by the presence of ‘B’ symptoms, cytopenias and bulky disease.…”

“…Among stage IV patients, 32% have BM involvement (3). However, the value of BMB in the staging of patients with HL is controversial, because the BM involvement in patients with prebioptic stage IA or IIA has been shown to be very rare in most series (4). In addition there is no consensus about the impact of BM infiltration on prognosis (5, 6).…”

“…disseminated involvement of an extra‐nodal organ. However, the incidence of BMI in patients otherwise staged as clinical stage (CS) IA or IIA has been shown to be < 1% in most series (Macintyre et al , 1987; Doll et al , 1989; Munker et al , 1995; Spector et al , 1997). In CS IV HL, marrow infiltration has previously been thought to confer a relatively poorer prognosis over patients without BMI (Rosenberg, 1971; Specht & Nissen, 1988).…”
