Protective Effect of Denosumab on Bone in Older Women with Primary Hyperparathyroidism

“…The finding that the effect of denosumab in terms of ΔALP is more evident in individuals with PHPT than in those with PO suggests that denosumab‐related reduction of bone turnover may be greater in individuals with PHPT than in those with PO. In individuals with PHPT, the greater bone turnover and consequent bone damage are thought to be mainly due to the excessive activation of RANK/RANKL pathway . On the basis of these considerations, the present finding of a dramatic positive effect of denosumab on BMD in individuals with PHPT could be considered somewhat expected.…”

Individuals with PHPT have low bone mineral density (BMD) and high risk of fracture because of the PTH‐mediated activation of the RANK/RANKL pathway . Therefore, denosumab could be an effective therapy for reducing bone damage in PHPT, but no studies have been performed to explore its possible use in individuals with PHPT with osteoporosis.…”
