Bony exostoses developed subsequent to free gingival grafts: case series

Abstract: Bony exostosis (BE) is described as a benign localised overgrowth of bone of unknown aetiology. Buccal bony exostosis (BBE) development secondary to soft tissue graft procedures has been reported in a small number of cases. The dental literature describes BBE development also at sites where free gingival grafts (FGG) have been used to increase the amount of gingiva. The following case series describes BBE development at nine sites (five cases) at which FGG was performed to increase the width of the attached gi… Show more

“…This condition may be explained, in part, by a similar condition that occurs around natural teeth. It has been argued that bone trabeculae around natural teeth restored or not with prosthetic restorations (i.e., fixed partial dentures, cantilevers, or removable partial dentures) can be reinforced in response to heavy occlusal forces 42‐45 . This bone adaptation, denominated buttressing, could be linked to chronic irritation and functional stresses caused by occlusion.…”

“…The occurrence of adverse effects and/or postoperative complications during the post-surgical period was reported in one trial. 16 Heitz-Mayfield et al 16 reported the loss of three (6.2%) implants (two TPS implants and one SLA implant) during the initial phase of healing, but of the remaining implants (45), no other implant was lost at the end of the study.…”

“…Benign osseous proliferation beneath posterior fixed partial denture pontics have also been reported in the dental literature 15 16. Another possible mechanism suggested by Chambrone and Chambrone5 is that genetic factors, functional stresses and chronic irritation may also act as aetiological factors for the development of exostoses.…”

“…4.2a-c ). The reduced base of literature suggests that periosteal trauma/fenestration is probably the primary maincausing agent linked with exostosis development in grafted sited by FGG [ 16 ]. Likewise, and as explained in Chap.…”
