A bronchobiliary fistula (BBF) is a rare abnormal communication between the biliary tree and bronchial system. The majority of cases are the result of biliary obstruction or injury, with the major symptomatology of cough and biliptysis. The initial management of BBFs is variable but aims to decompress the biliary system allowing for diversion and passive healing of the fistula tract. Definitive management is with surgical fistulectomy. New minimally invasive therapeutic approaches utilizing endoscopic or percutaneous methodology have been described with some success. We present the successful treatment of a BBF that developed secondary to chemotherapy-induced biliary stricturing (CIBS) with a novel percutaneous embolization approach using a vascular plug and liquid embolic agent.

Bronchobiliary fistula (BBF) is an abnormality between the bronchial tree and biliary tract system. Misdiagnosis and delayed treatment might have a high mortality rate. Although some characteristics relevant to BBF have been introduced, difficulties and controversies in diagnosis and treatment need further investigation. In the present study, we summarize the diagnostic and surgical treatment experience of a 65‐year‐old male patient who presented with an irritating cough and biliptysis. As he had a history of aortic valve replacement surgery and schistosomial cirrhosis, the provisional diagnosis was BBF, which might be secondary to a foreign body reaction or liver cirrhosis and chronic cholecystitis and cholelithiasis. During the surgery, it was discovered that the right lobe of the liver was significantly atrophied with irregular contours, and the gallbladder was adjacent to the diaphragm with a fistula between the fundus of the gallbladder and the right semidiaphragm. These results confirmed that the BBF in this patient resulted from cholelithiasis associated with serious schistosomial cirrhosis. He was successfully treated with a right hepatic lobe resection and Roux‐en‐Y hepaticojejunostomy. The diaphragm defect was repaired by imbrication of the repaired liver and lower lobe's edges. A high index of clinical suspicion can be generated by patients who present with recurring irritating cough and production of bile‐like sputum. The manifestations and responses to conservative treatment are crucial in clinical judgment, and surgical intervention is the primary treatment choice when other approaches have failed or there are complications from the underlying disease.

Bronkobiliyer fistül, biliyer sistem ile bronş sistemi arasında defektif geçiş olması ile karakterize nadir görülen bir durumdur. Daha çok karaciğer kist hidatik veya amibik hastalığı, kolelitiazis, koledokolitiazis, peptik ülser, malignite, konjenital malformasyonlar ve cerrahiye sekonder bir komplikasyon olarak ortaya çıkmaktadır. Tedavisinde cerrahi yöntemlerinde, morbidite ve mortalite oranı yüksek olduğundan günümüzde daha az invaziv yöntemler tercih edilmektedir. Bu yazıda karaciğer kist hidatiğine bağlı gelişen bronkobiliyer fistüllü olgunun, endoskopik retrograd kolanjiopankreatografi yönte-mi ile hem tanı konulmasını hem de tedavi edilmesini sunuyoruz. Anahtar kelimeler: Bronkobiliyer fistül, endoskopik retrograd kolanjiopankreatografiBronchobiliary fistula is a rare condition characterized by an abnormal connection between the biliary and bronchial system. The condition can be caused by hydatid cysts, amoebic disease, cholelithiasis, choledocholithiasis, peptic ulcer, malignancies, congenital malformations and surgery. The risk of morbidity and mortality inherent with surgical intervention makes less invasive methods preferred. Herein, we report a case in which endoscopic retrogradcholangio pancreatography was used for the diagnosis and treatment of a bronchobiliary fistula.
