Caffeine Citrate

Abstract: Caffeine citrate was generally well tolerated by neonates in clinical trials and it decreased the incidence of apnoea of prematurity compared with placebo. It has demonstrated similar efficacy to theophylline, but is generally better tolerated and has a wider therapeutic index. Caffeine citrate should, therefore, be considered the drug of choice when pharmacological treatment of apnoea of prematurity is required.

“…Various references mention the gastrointestinal side effects of the methylxanthines, including feeding intolerance, aspirates, and gastrointestinal irritation presenting as bloody aspirates. 4,5,9,10 Apnoea also has been found to be associated with low birth weights below 1 000 g 11 and has been observed in as many as 80% of infants under 1 000 g. 12 Four patients (n = 31) developed apnoeic attacks during the study period.…”

“…Intravenous access was also problematic in many neonates who were admitted only for kangaroo mother care (KMC). Caffeine occurs naturally in many plants and has been used for over 20 years to treat apnoea of prematurity 5 and may be a suitable alternative.…”

“…9 The in vivo translation of these findings in preterm infants is still unknown. Although caffeine citrate has a wide therapeutic index for the treatment of apnea of prematurity and no routine serum levels are recommended, 11-13 factors such as the empiric dose adjustment on the basis of clinical response and the variations in renal clearance caused by acute clinical events could lead to serum caffeine levels outside of the recommended therapeutic levels, potentially changing the pattern of binding to ARs. 14-17 …”

“…Caffeine citrate is a central nervous stimulant and helps to restore alertness. Pharmaceutically, caffeine citrate is used in the treatment of apnea in newborns; also it is the component of many of the analgesic formulations, where it demonstrates a synergistic effect with those analgesics (Comer et al, 2001; Sawynok, 1995). …”
