Caloric Restriction and Ketogenic Diet Therapy for Epilepsy: A Molecular Approach Involving Wnt Pathway and KATP Channels

Abstract: Epilepsy is a neurological disorder in which, in many cases, there is poor pharmacological control of seizures. Nevertheless, it may respond beneficially to alternative treatments such as dietary therapy, like the ketogenic diet or caloric restriction. One of the mechanisms of these diets is to produce a hyperpolarization mediated by the adenosine triphosphate (ATP)-sensitive potassium (K ATP) channels (K ATP channels). An extracellular increase of K + prevents the release of Ca 2+ by inhibiting the signaling … Show more

“…The KD provides ketone bodies as a brain energy source instead of glucose, stabilizing synapses and improving brain energy reserve which leads to a neuronal function enhancement. The CR also decreases glucose sensitivity, but without producing ketone bodies [163][164]. The KD should be recommended with caution, more evidence is needed to support this type of therapy, there is a concern in patients with T1DM because of the lipid profile provided by the diet, as well as in patients with T2DM with increased cardiovascular risk, therefore, each diet should be tailored for each patient [165].…”

“…The Wnt/β-catenin pathway participates not only in neuronal synchrony regulation. But also in NMDA receptor modulation, which, as previously described, plays an important role not only in epilepsy but also in epileptogenesis [25,26].…”

“…Among the genes altered, the Abcc8 mRNA levels are downregulated and Kcnj11 levels tend to increase (Kundap et al, 2020). This situation is supported by the observation that epilepsy is positively modulated with diets such as the ketogenic diet or caloric restriction which produce a hyperpolarization mediated by the K ATP channels (Rubio et al, 2020). These data show the complex interaction of SUR1 proteins with ion channels and the importance of their study during brain disease.…”
