Can specific vessel‐based papulopustular lesions of Behçet's disease be differentiated from nonspecific follicular‐based lesions clinically?

Abstract: Clinical examination may not be sufficient to predict the dermatopathologic pattern in all lesions. Papulopustular lesions with no specific clinical and histopathologic features may create problems in the diagnosis of Behçet's disease.

“…29 According to the ISG diagnostic criteria, acneiform lesions in postadolescent patients who are not on systemic corticosteroid treatment are interpreted as a sign of Behçet's syndrome. 16 In studies evaluating the papulopustular lesions of Behçet's patients, the mean age of the patients is greater than 30 years.. [29][30][31] Acne vulgaris also can be observed in 20% of men and 35% of women over the age of 30, according to one study. 32 Postadolescent acne persisting beyond the age of 25 years usually is seen in women during the last week of the menstrual cycle.…”

“…Similarly, the histologic fi nding in papulopustular lesions may overlap with those of bacterial folliculitis, but the fi nding of lymphocytic vasculitis should only be present in BD lesions [ 16 ]. In any case, the clinical separation of vessel-based papulopustular lesions of BD cannot be reliably distinguished from nonspecifi c folliculitis [ 17 ].…”
