Epidemiology of Leptospirosis in Africa: A Systematic Review of a Neglected Zoonosis and a Paradigm for ‘One Health’ in Africa

BackgroundLeptospirosis is an important but neglected bacterial zoonosis that has been largely overlooked in Africa. In this systematic review, we aimed to summarise and compare current knowledge of: (1) the geographic distribution, prevalence, incidence and diversity of acute human leptospirosis in Africa; and (2) the geographic distribution, host range, prevalence and diversity of Leptospira spp. infection in animal hosts in Africa.MethodsFollowing Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines, we searched for studies that described (1) acute human leptospirosis and (2) pathogenic Leptospira spp. infection in animals. We performed a literature search using eight international and regional databases for English and non-English articles published between January 1930 to October 2014 that met out pre-defined inclusion criteria and strict case definitions.Results and DiscussionWe identified 97 studies that described acute human leptospirosis (n = 46) or animal Leptospira infection (n = 51) in 26 African countries. The prevalence of acute human leptospirosis ranged from 2 3% to 19 8% (n = 11) in hospital patients with febrile illness. Incidence estimates were largely restricted to the Indian Ocean islands (3 to 101 cases per 100,000 per year (n = 6)). Data from Tanzania indicate that human disease incidence is also high in mainland Africa (75 to 102 cases per 100,000 per year). Three major species (Leptospira borgpetersenii, L. interrogans and L. kirschneri) are predominant in reports from Africa and isolates from a diverse range of serogroups have been reported in human and animal infections. Cattle appear to be important hosts of a large number of Leptospira serogroups in Africa, but few data are available to allow comparison of Leptospira infection in linked human and animal populations. We advocate a ‘One Health’ approach to promote multidisciplinary research efforts to improve understanding of the animal to human transmission of leptospirosis on the African continent.

Abstract. The epidemiologic status of leptospirosis in Egypt has not been well defined because of difficulties in disease diagnosis. A retrospective study was conducted to detect leptospiral antibodies among undiagnosed acute febrile illness (AFI) and hepatitis cases. Approximately 16% of both AFI (141/886) and acute hepatitis (63/392) cases showed seroreactivity to Leptospira IgM by ELISA and microscopic agglutination test (MAT). Canicola, Djasiman, Grippotyphosa, Pyrogenes, Icterohemorrhagiae, and Pomona were the most commonly reactive serovars among patients with AFI. Djasiman, Grippotyphosa and Icterohemorrhagiae were the most reactive among patients with acute hepatitis. This study represents the first systematic report of Leptospira associated with patients with AFI and hepatitis in Egypt. Physicians need to have increased awareness about the importance of leptospirosis in the differential diagnosis of AFI and acute hepatitis in Egypt. In addition, laboratory capacity should be developed at fever hospitals to diagnose leptospirosis.

Summary Throat swabs, faecal swabs, blood, urine and kidneys from 50 stray dogs and 50 house cats were examined. Throat swabs revealed Strept. pyogenes in dogs and cats at an incidence of 2% and 8%, respectively; Strept. faecalis and Staphylococcus aureus were each isolated from 6% of dogs. Faecal swabs yielded S. typhimurium (4%) and S. newport (2%) from dogs, while S. anatum and S. paratyphi B were each isolated from 2% of cats. The agglutination lysis test on the sera of cats was negative, whereas dogs reacted positively against L. canicola and L. icterohaemorrhagiae in 10% and 2% of animals, respectively. Attempts to isolate leptospires from urine and kidneys were unsuccessful. Zusammenfassung Die mögliche Rolle streunender Hunde und Katzen in der Epidemiologie von einigen humanpathogenen Bakterienarten in Ägypten Rachen‐ und Analabstriche sowie Blut, Harn und Nieren von 50 streunenden Hunden und 50 Hauskatzen wurden untersucht. Die Rachenabstriche wiesen in 2% der Fälle bei Hunden und 8% der Fälle bei Katzen Streptococcus pyogenes auf. Streptococcus faecalis und Staphylococcus aureus wurden isoliert von jeweils 6% der Hunde. Die Analabstriche von Hunden enthielten Salmonella typhimurium (4%) und S. newport (2%), während aus denen von Katzen S. anatum und S. paratyphi B, jeweils mit 2%, isoliert wurden. Der Agglutinations‐Lysis‐Test war bei den Sera der Katzen stets negativ, bei den Hunden reagierten gegen Leptospira canicola und L. icterohaemorrhagiae 10% resp. 2% der Tiere positiv. Versuche, Leptospiren aus Harn oder Nieren zu isolieren, verliefen nicht erfolgreich.
