Carcinoma arising in eccrine spiradenoma

Abstract: Two patients had carcinoma arising in eccrine spiradenoma. In both patients, recent changes in a long-standing cutaneous nodule prompted excision, and microscopic examination disclosed carcinoma intimately associated with and apparently arising from typical benign eccrine spiradenoma. One patient had lymph node metastasis.

“…In the histopathologic examination of MES, two distinct components are seen: typical benign eccrine spiradenoma and carcinoma with areas of transitions 8 . The carcinoma may show squamous differentiation or sarcomatous change 1,7,14 . Microscopic examination in our case showed only carcinoma, including proliferation of solid masses of tumor cells with large hyperchromatic nuclei and frequent atypical mitoses, invasion of surrounding connective tissue, loss of basement membrane, and squamous differentiation.…”

“…The clinical course of MES varies in the reported cases. Metastasis to the bones, lungs, brain, and lymph nodes have been described 2,7,14,16,20 . Various adjuvant treatment modalities including chemotherapy, radiation therapy, and lymph node dissection have also been used 3,10,21 .…”

“…Metastasis of bones, lungs, brain, and lymph nodes have been described. [910] Close postoperative follow-up is very important to monitor local recurrence, or regional or distant metastasis. Wide local excision seems to be the adequate treatment for MES.…”

“… 33 Although exceptionally rare, carcinomatous transformation has been reported in a preexistent long‐standing benign eccrine spiradenoma. 34 On the other hand, one must bear in mind the case of a 58‐year‐old man with growing tumors of the scalp (presented to the Los Angeles Dermatological Society on October 8, 1958) where total scalp resection was proposed. “This procedure was accomplished and followed by full thickness graft, with good healing.…”
