Cardiac arrhythmias as the initial manifestation of adult primary Sjögren's syndrome: a case report and literature review

Abstract: Two middle-aged female patients presenting with heart palpitation and electrocardiogram revealed complex cardiac arrhythmias. A review of systems was positive for dry mouth and transient arthralgia, while laboratory and instrumental tests enabled us to make the diagnosis of primary Sjögren's syndrome (pSS). Cardiac electrophysiology revealed atrioventricular node dysfunction and impaired intraventricular conduction. Prednisone therapy induced a significant improvement in symptoms and electrocardiographic readi… Show more

“…Fortunately, cardiac manifestations such as pericarditis, pulmonary hypertension and cardiomyopathy are very rare, affecting <1% of pSS patients, respectively [ 207 ]. Cardiac rhythm disturbances have been described, secondary to ionic disorders, dysautonomia or direct impairment of the electrical conduction system of the heart [ 224 , 225 ].…”

“…We found that compared with the ICR mice, the submandibular gland tissue of NOD mice model had histology of acinar destruction, basement membrane changes, and lymphocyte infiltration. Studies have shown that SS, in addition to epithelial dysfunction, has several extraglandular manifestations associated with lymphocyte infiltration and B cell overactivity in other organs [40][41][42]. Numerous studies have shown that aquaporin AQP5 is a crucial target for determining salivary gland secretion and tissue formation [43][44][45].…”

“…160 Cardiac involvement is uncommon in pSS but atrioventricular block and complex cardiac arrhythmias have been described. 161,162 In one study, echocardiograms were performed in 107 consecutive subjects with SS and 112 matched healthy controls. 163 Among the SS cohort, the incidence of mitral, tricuspid, and aortic regurgitation was higher than that in controls (p < 0.001, p ¼ 0.02, and p < 0.007, respectively) and left ventricular mass index was increased in SS (p < 0.01).…”
