Cardiac cobaltism: a rare complication after bilateral metal-on-metal total hip arthroplasty

Abstract: This is one of the first documented cases of metal-on-metal (MoM) total hip arthroplasty cobalt cardiac toxicity that has led to patient death. A 69-year-old female presented to our institution with cardiac failure secondary to cobalt toxicity resulting from bilateral MoM total hip arthroplasty. Her presenting metal ion levels were a cobalt level of 199 ppb and a chromium level of 77 ppb. She underwent bilateral femoral revisions to remove the source of cobalt. On postoperative day 7, the patient sustained a c… Show more

“…Increased serum cobalt and chromium metal ions, ALTR, osteolysis, and failure rates 2 to 3 times that associated with metal-on-ultra-high-molecular-weight polyethylene have been well described [13] , [15] . In extreme cases, cobalt poisoning can result in neurologic injury, such as permanent hearing and vision loss, and even in death from cardiac cobalt toxicity [14] , [16] . The potential for increased local and systemic cobalt and chromium metal ions and ALTR exists whenever there is corrosion of a CoCr implant [16] .…”

“…Although causation is suspected, our patient's cardiac biopsy was improperly processed for presence of cobalt. Positive biopsy is not a requirement for diagnosis, although other case reports of cardiomyopathy associated with occupational [ 15 ] and prosthetic cobalt exposure [ 16 ] have shown positive cobalt deposition in the myocardium. In our patient's case, the patient had a precipitous decline in serum cobalt levels and significant improvement in cardiac function following his revision operations, supporting the hypothesis that his cardiomyopathy was associated with elevated serum ion levels.…”
