Cardiac surgery using a single thoracic port—current status and future directions

Abstract: Cardiac surgery is almost universally performed through a median sternotomy, an approach which is painful, unestethical, and prone to life-threatening infections. Minimally invasive cardiac surgery has tried to avoid problems associated with full sternotomy for many years. Recently, uniportal thoracic surgery was shown to be very advantageous when compared to standard thoracotomy and classical video assisted thoracic surgery (VATS). Despite very good results in lung surgery, cardiac surgery through a single th… Show more

“…The results suggest that MICS is associated with the potential benefit of improved cosmesis, less pain, shorter recovery duration, and faster return to normal activities, without compromising the safety of the procedure [ 12 , 13 ]. Furthermore, MICS has been used in the treatment of various cardiac lesions, and has become the preferred treatment standard in some heart centers [ 14 17 ]. MICS is becoming important in the field of cardiac surgery, and be applied globally.…”

“…Since its inception cardiac surgery has been performed with sternotomy which is usually accompanied with pain, not really aesthetic and prone to life threatening infections. The incidence of deep sternal wound infection is 0.5-6% and related hospital mortality is 7-35 % [1]. There is also a possibility of sternal instability without an accompanying infection [2].…”

“…Carpentier and Mohr performed the first total robotic mitral valve repair in 1998 [2]. With these first approaches many minimal invasive incisions were postulated; J, inverted J, T, inverted T, V shaped inferior and superior partial sternotomies, right parasternal incision, right and left mini thoracotomy, right infra axiliary thoracotomy [1,2]. With the encouragement of the minimal invasive surgery improvements were made to minimize the inflammatory effects of extracorporeal circulation, the cannulas were made smaller, and carbon dioxide was employed to the surgical field to lessen the occurrence of air embolism.…”

“…Reducing surgical trauma is one of the obvious ways to reduce perioperative risks and improve surgical techniques [1][2][3], which is also very positively perceived by patients [4,5]. At the same time, less traumatic techniques are accepted for use only if the therapeutic goal is effectively achieved with a reliable result, preferably remote [6,7].…”
