Cardiovascular Prevention: Migrating From a Binary to a Ternary Classification

Abstract: Migrating from a binary approach to risk assessment to a ternary model of disease identification allows for individualized, optimal disease management. Redefining the disease/inflammatory approach has been proven to identify, stabilize, and regress atherosclerosis while adding understanding to the progression of vascular disease. Our previously published results show the beneficial effect of comprehensive, evidence-based management on subclinical atherosclerosis and vulnerable plaque. We argue that this approa… Show more

“…The current binary ASCVD risk classification system defines primary prevention as preventing adverse ASCVD events in individuals with no history of a MACE and secondary prevention as preventing recurrent adverse ASCVD events in individuals who have previously suffered a MACE. Doneen et al (2020) proposed a new ternary classification system to enhance individualized care and risk reduction. Under this classification system, primary prevention includes individuals without existing ASCVD (Lamon & Hajjar, 2008).…”

“…This approach aims to reduce arterial inflammation and stabilize plaque to prevent progression and adverse ASCVD events. Finally, tertiary prevention includes individuals with a history of MACE, with the treatment goal focusing on stabilizing plaque, reducing arterial inflammation, and preventing future events (Doneen et al, 2020). This paradigm change requires transforming one's mental model and approach to the individual patient's risk assessment.…”

“…Tertiary prevention was given to subjects who proved they had arterial disease with interventions or events. This classification aided therapy decisions such as low-dose aspirin ( 270 ). The high degree of management compliance is partially attributed to the patient's visualization of the plaque ( 271 ).…”
