Caseous calcification of the mitral annulus on MDCT: a rare intracardiac mass

Abstract: Caseous calcification of the mitral annulus is a rare form of chronic degenerative process in the mitral valve fibrous ring with a mass-like appearance that has to be in the differential of the radiologist and cardiologist. We present the case of an 82-year-old woman in whom a tumor-like calcified mass in the posterior side of the mitral valve annulus was detected at echocardiography, and the diagnosis of caseous calcification was confirmed on multidetector computed tomography (MDCT). The diagnostic features o… Show more

“…При выполнении МСКТ сердца ККМК представляет собой гиперинтенсивное округлое или овальное кальци нированное образование в области задней ветви митраль ного кольца с четкими краями, имеющее низкую степень накопления контрастного вещества [17]. МРТ сердца является важным методом, позволяющим проводить дифференциальную диагностику с другими интракарди альными образованиями.…”

“…On CT, CCMA is reported to appear as a round or semilunar sharply marginated mass with a hypo-dense or hyper-dense center and a calcified peripheral rim ( 8 ). The absence of enhancement after the administration of contrast agent would be useful for differentiating CCMA from other cardiac masses, such as tumors ( 9 ). In our case, previous echocardiography clearly showed an echo-dense mass and that was sufficient for a diagnosis of CCMA; the diagnosis was confirmed by CT.…”

“…The symptoms, when present, correspond to palpitations, dyspnea, and syncope ( 11 ) . The prognosis of caseous degeneration of the mitral annulus is good, especially in patients who are asymptomatic, although some patients develop severe symptomatic valvular dysfunction; in the latter group of patients, the prognosis is poor and surgery should be considered ( 9 , 12 ) .…”
