Cataract surgery in small pupils

Abstract: This paper presents the review of historical aspects and the current state-of-the-art in various pupil dilatation methods to be used in cataract surgery. The surgical algorithm in managing small pupil cases should include topical and intraocular mydriatics, appropriately selected viscosurgical device and mechanical dilatation with instruments, iris hooks, and/or pupil expanders.

“…Local stretching of the iris with iris retractors led to the damage of rim pigment. Alternatively, different kinds of iris retracting devices with smooth dilation principle can be used (Malyugin ). But the application of iris expanders is limited in the youngest children due to the small size of their eyes (anterior chamber depth, white‐to‐white distance, etc.…”

“…Patients with pupillary diameter above 5 mm were included in group 1, and those below 5 mm were included in group 2. The reference pupillary diameters for grouping were made according to the narrow and normal pupillary width specified in the referenced studies (17)(18)(19). Group Visual acuity, intraocular pressure and anterior chamber depths were evaluated in both groups before and after surgery.…”
