Cataract surgery with a small-aperture intraocular lens after previous corneal refractive surgery: Visual outcomes and spectacle independence

Abstract: Patients who have had previous refractive procedures often require cataract surgery at an early age; however, postoperative results are compromised by corneal aberrations and difficulties achieving a desired refractive endpoint. We describe the use of the IC-8 small-aperture intraocular lens (IOL) in 3 patients who had previous laser in situ keratomileusis (LASIK). Standard cataract surgery was performed in all cases. The small-aperture IOL was implanted in the patient's nondominant eye, and a clear aspheric m… Show more

“…The implantation of the small-aperture IOL we used, the AcuFocus IC-8, and a monofocal or bifocal IOL in the fellow eye has been performed successfully in several studies. 7 , 8 , 38 , 39 The IC-8 features a black mask with a diameter of 3.23 mm and a central aperture of 1.36 mm, that enhances the depth of focus using the pinhole effect. Studies reported good results for far and intermediate distance and a low level of photic phenomena.…”

“…We targeted a slightly myopic refraction in the non-dominant left eye for the IC-8 IOL as described in previous reports and recommended by the manufacturer. 4 , 5 , 6 , 7 , 8 The postoperative myopic refractive error created a mini-monovision additionally enhancing uncorrected near and intermediate visual acuity.…”

“…13 However, to our knowledge, no studies on the implantation of the AT Lisa 939M IOL in eyes with previous history of keratorefractive procedures have been published. The IC-8 IOL has recently provided patients with corneal irregularities resulting from trauma 14 and previous kerato-refractive procedures 15,16 an extended range of vision while avoiding the problems associated with monovision and multifocal IOLs; patients generally achieve spectacle independence and good vision at all distances, [14][15][16] and CS and stereopsis 16 are well maintained. This case series reports the outcomes of unilateral IC-8 IOL implantation in two patients with history of bilateral myopic RK and one patient with history of bilateral myopic RK and AK, and bilateral implantation of AT Lisa 939M trifocal toric IOLs in a single patient with history of PRK and LASIK.…”
