CD5 Expression in a Lymphoma of the Mucosa-Associated Lymphoid Tissue (MALT)-Type as a Marker for Early Dissemination and Aggressive Clinical Behaviour

Abstract: Mucosa associated lymphoid tissue (MALT) developing in response to chronic infection or autoimmune stimuli has been recognized as a possible site of origin for a distinct type of B-cell lymphoma. While preferentially occurring in the stomach, MALT-type lymphomas can be found in virtually all organs. MALT-type lymphomas normally follow an indolent course, with a tendency to remain localized at their site of origin for a prolonged period of time. Histologically, MALT-type lymphomas are heterogeneous covering a c… Show more

“…The prognostic significance of T-cell antigen expression in B-cell non-Hodgkin's lymphomas remains somewhat controversial. CD5 expression in some series of extranodal marginal zone lymphoma 27,28 and de novo diffuse large B-cell lymphoma 22,23,29 has been associated with a clinically aggressive course and tendency for dissemination, although other studies have failed to support these findings. 30 Only one of five patients with clinical follow-up data in our study died of disease.…”

“…36 -38 Immunophenotyping typically shows expression of pan B-cell markers but no expression of CD23, CD10, and CD5; however, rare CD5-positive cases have been reported. 39,40 The majority of cases, but not all, express CD11c. 33 This indolent lymphoma is often hard to subtype because of its rarity, spectrum of cytomorphologic features that overlap with those of reactive processes, and lack of a distinctive positive immunomarker.…”

“…Typically, tumors express pan-B antigens (CD19, CD20, CD22, CD79a), but they lack CD5, CD10, CD23, and Bcl-1 expression. In rare cases, MALT lymphomas exhibit aberrant CD5 expression, which may be associated with a more aggressive clinical course [5]. Table 1 summarizes the immunophenotypic and genetic features of common small lymphocytic B-cell neoplasms included in the differential diagnosis of MALT lymphomas.…”
