Testing of Braden Qd Scale for Predicting Pressure Ulcer Risk in The Pediatric Intensive Care Unit

Children admitted to the Pediatric Intensive Care Unit (PICU) are at risk for pressure injury due to immobility and the challenge of positioning medical devices. However, a comprehensive instrument to assess pressure injury risk is still finite. This study aimed to test the validity and reliability of the Braden QD Scale. The design used was a Pearson correlation coefficients and Cronbach’s alpha (α). Fifty-one pediatric patients below age 18 who had been bedridden for at least 24 hours were engaged through a consecutive sampling method. The results showed assessments made by using the Braden QD Scale revealed that 88.2% of patients were at risk for a pressure injury. The validity test results of the Braden QD Scale ranged between 0.532 and 0.833. The reliability test results of the Braden QD Scale ranged between 0.756 and 0.834. The sensitivity value is 100%, specificity value is 40%. In general, the study finding suggests that the Braden QD Scale is a valid and reliable instrument for predicting pressure injury risk. Nurses may employ this tool to assess pressure injury risk in pediatric patients.

RESUMO Objetivo: adaptar transculturalmente e validar o instrumento Braden QD Scale para uso em neonatos no Brasil. Método: estudo metodológico de adaptação transcultural e estudo transversal observacional de validação, realizado entre dezembro de 2017 a agosto de 2021. Participaram 10 especialistas, 38 enfermeiros e 105 recém-nascidos. O processo de adaptação transcultural envolveu a tradução inicial, síntese, retrotradução, comitê de especialistas, pré-teste e aprovação da versão adaptada pela autora do instrumento original. A validação verificou as propriedades psicométricas de validade, confiabilidade e consistência interna, a partir da aplicação do instrumento adaptado por dois avaliadores, de forma simultânea e independente, e avaliação temporal a partir de vídeo de cinco neonatos em dois momentos distintos. Os testes estatísticos foram o Índice de Validade de Conteúdo, alfa de Cronbach e coeficiente Kappa. Resultados: o processo de tradução da Braden QD scale resultou na versão em português adaptada para a cultura brasileira. O índice de validade de conteúdo do comitê de especialistas foi ≥0.90 e do pré-teste foi ≥0.80. Na confiabilidade interobservador todos os itens obtiveram coeficiente Kappa >0,90. O alfa de Cronbach do avaliador 1 foi de 0,773 e do Avaliador 2 foi de 0,769, sendo confiável o alfa de Cronbach >0,6. Na concordância intraobservador a média dos escores, na prática, não foram diferentes. Conclusão: o instrumento foi adaptado transculturalmente para uso em neonatos e crianças no Brasil. A versão brasileira apresentou níveis estatísticos de validade e confiabilidade, mostrando-se válida para uso em neonatos no Brasil.

Nutrition status plays a critical role in pressure injury (PI) healing and yet the available literature, especially in pediatric patients, is limited. Critically ill pediatric patients are at an increased risk of skin integrity compromise and PI development. Adequate nutritional intake can often be challenging to achieve in this population and immobility and illness present additional obstacles to maintaining skin integrity in this vulnerable population. Despite the unique nutritional challenges and needs of this group, there is no standardized approach to macro-and micronutrient management and monitoring. Here, several key vitamins and minerals believed to play a role in PI healing are discussed and an approach to nutritional management and monitoring for PI healing in pediatric patients is proposed. Registered dietitians (RD) are essential to assess individual patient macro and micronutrient requirements, to identify gaps and make recommendations to optimize nutritional therapy that may exist and impact wound healing. We used a scoping review to focus on the interplay of nutrition and PI healing and inform a multidisciplinary approach to PI identification and management. Through this review, we propose a strategy for the nutritional management of pediatric patients <30 kg at risk for and who present with PI.
