CE: How to Predict Pediatric Pressure Injury Risk with the Braden QD Scale

Abstract: The Braden QD Scale is a conceptually based, pediatric-specific, risk assessment instrument that reliably predicts both immobility-related and medical device–related pressure injuries in the pediatric acute care environment. A revision and simplification of the commonly used Braden Q Scale, the Braden QD Scale can be used to assess risk among the wide range of infants, children, and adolescents commonly treated in acute care environments. As part of a comprehensive program to prevent hospital-acquired pressure… Show more

“…11 There are several weaknesses in the Braden Q Scale, including an inability to identify medical device-related pressure injury, as the scale was originally applied for children aged 3 months to 8 years, and inapplicable for patients with congenital heart disease. 16 Flaws within the Braden Q Scale prompted Quigley and Quartrano to modify and simplify it to the Braden QD Scale. The Braden QD Scale offers several advantages over the Braden Q Scale.…”

“…Como critério de inclusão os enfermeiros deveriam trabalhar na instituição há pelo menos seis meses. Cada enfermeiro recebeu o instrumento final adaptado e o treinamento para aplicar o protocolo de pesquisa conforme recomendação das autoras do instrumento original 17 . As avaliações foram realizadas pelos enfermeiros duas vezes por semana, de forma simultânea e independente, iniciando-se no momento da admissão do RN na pesquisa e sendo finalizada após transferência, alta ou óbito do mesmo.…”

“…Routine and thorough skin assessments begin with the patient's nurse, is shared between all members of the clinical team and are crucial in PI prevention and management in pediatric patients. Prevention of PIs starts with a thorough assessment of the patient and their skin integrity upon admission and continues routinely at serial intervals to identify injuries and accurately determine the risk of developing a hospital-acquired PI (53). These evaluations range from detailed physical assessment to review of medical records and include identification and stratification of risk (53).…”

These evaluations range from detailed physical assessment to review of medical records and include identification and stratification of risk (53). Risk assessments are performed at a frequency based on unit standards, patient diagnosis, acuity, mobility and, other aspects of the patient's condition that may predispose them to a PI (53).…”
