Cephalometric changes in growing patients with increased vertical dimension treated with cervical headgear

Abstract: The vertical dimension was not significantly altered after cervical headgear treatment although the anterior facial height was higher at the beginning of treatment. There was significant counterclockwise rotation of the mandible, and clockwise rotation and distal displacement of the maxilla after treatment.

“…From the results of this paper we can state that a good occlusion is very important to maintain the stability of the kinetic chain of the body. Particularly, as we already showed, the position of premolars plays an important role in the stability of occlusion [36], and early orthodontic treatment could prevent the development of complex malocclusions, especially cross-bites [34,[37][38][39][40][41].…”

“…Different kinds of malocclusions can be corrected with different approaches and appliances, but in some cases, it is necessary to extract healthy teeth in order to reduce the convexity, and this is extractive therapy [7,8]. In young patients with a dolichofacial pattern, cervical headgear is the best approach to correct the class II malocclusion by orthopedic maxillary alteration, with a superb control of the vertical dimension [13]. In very difficult cases, in which the skeletal discrepancy is too much, or in protocol surgery first [14], the condition could be solved with orthognatic surgery which allows us to treat severe conditions [15][16][17]; but surgery exposes the patients to risks [18][19][20].…”

“…The outcome of this study could be different if the groups were treated in different growth stages. However, it is noteworthy that in many studies on CH treatment, the initiation of treatment has been done between 8.5 and 10.5 years, before or during the initiation of the second mixed dentition (4,5,11,12).…”

“…For this reason, the use of CH is not recommended in patients with vertical growth pattern, even though there are reports where the use of CH with vertical growth pattern did not worsen the posterior growth (11). On the contrary, anterior rotation of the mandible has been reported (11,12).…”
