Cervical intervertebral disc calcification with extreme lateral herniation in a child: T2-weighted signal intensity of the involved disc can be restored to normal

Abstract: Cervical IDC combined with extreme lateral herniated calcification is extremely rare in children. The recovery of signal intensity of intervertebral disc on MRI may provide further support to the feasibility of conservative treatment of IDC.

“…Coordinate with previous reports [16, 3942], narrowing of the involved intervertebral space, flatting and wedging of adjacent vertebral body were observed in the current case at 9-year follow-up. IDC with OPLL in children seemed benign and self-limiting.…”

“…Nineteen months after initial diagnosis, restoration of T2-weighted signal intensity of C2/3 and C3/4 discs was observed in the current case through MRI. Similar change was reported by Liu [16], who reported a calcified disc restored to normal T2-weighted signal intensity at 2-year follow-up for a 10-year-old girl. The mechanisms of “rehydration” of the calcified discs are still unclear.…”

“…Dans la littérature, on ne relève que dix cas d'enfants traités chirurgicalement dont la plupart avant 1980 [ 16 ]. Actuellement l'attitude est beaucoup plus conservatrice accompagnée d'une surveillance de l'évolution symptomatique [ 17 ]. Malgré un très bon pronostic, un cas clinique a été décrit en 2002 mettant en évidence la possibilité de survenue de séquelles au long cours au sein de cette pathologie réputée comme bénigne.…”

“…Paediatric idiopathic intervertebral disc calcification (PIIDC) is rare with less than 400 cases reported since the first published case in 1924 by Baron et al [1][2][3] The aetiology is unknown. 1 Several hypothesises have been described including minor trauma or a viral infection-induced vasculitis leading to a decreased nutrient supply of the disc.…”
