Abstract: Although overall RBC distributions have decreased over time, the proportion of O units has increased substantially.

“…As patient blood management becomes more widespread, there has been an international decline in the demand for RBC products . Hirani et al reported that there had been a 21% reduction in demand between 2012 and 2015 in Australia.…”

“…The counter-arguments are:The expected increase of blood donations of currently deferred donors is negligible, adding only 2.1%, 1.0% [3, 9] and 0.3% [9, 22] additional donors with no deferral, 1 year and 5 years deferrals respectively. In Israel, like in other developed countries, a Patient Blood Management Program exists, causing a drop in the usage of blood units and components [30, 31] in times of peace. When an urgent need to increase the national blood inventory arises, it can be achieved by safer means such as increased recruitment of members of the large population group who are not at high risk, who have relatively lower donation propensities.…”

“…For example, while the combined distributions of B1 and AB1 RBC units as a proportion of the total number of RBC distributions have decreased by 2.6% between 2010 and 2014 at several American and national or provincial blood collectors, the proportion of O-RBC distributions has increased by 10.4% over the same time period at these collectors. 1 Perhaps similar countercurrent trends in antigennegative RBC distributions have also occurred; these are units that hospitals order with a specific antigen phenotype for patients with RBC antibodies or for prophylactic antigen matching in those at risk of becoming alloimmunized, such as patients with sickle cell disease (SCD). [3][4][5] This study was performed to determine if there are …”

“…1 This is likely in large part due to the increasing adoption of hospitalbased patient blood management programs that tend to decrease the number of RBC units transfused based on a growing body of evidence that does not favor a liberal transfusion strategy for most patients. 2 However, within the larger trend of decreasing overall distributions, some countercurrent trends have developed.…”
