Changes in Community-Associated Methicillin-Resistant Staphylococcus aureus Skin and Soft Tissue Infections Presenting to the Pediatric Emergency Department

Abstract: The prevalence of CA-MRSA skin infections, specifically abscesses, has significantly increased at our institution from 2003 to 2008. The antibiotic resistance patterns have not significantly changed. The most common anatomic location for CA-MRSA abscesses continues to be the buttocks, but more children are presenting with multiple abscesses in a wider variety of anatomic locations.

“…14,29 Recent reports of clindamycin susceptibility rates in the United States have varied widely, with many studies identifying resistance rates of less than 10%. 15,30,31 Other reports include clindamycin resistance exceeding 30% to 50%. 11,12 It is difficult to make direct comparisons between these studies due to significant variability in study dates, target populations, and methodology.…”

“…A high burden of disease associated with MRSA infection has been reported in preschool-aged children. 2,14,30 Toddlers have a high incidence of MRSA buttock and perineal abscesses that may account for a portion of the higher SSTI rate. 15,30,36 Additionally, a recent meta-analysis suggested that the communityassociated MRSA epidemic may have peaked earlier in children than in adults.…”

“…This bacterium is a significant cause of surgical or traumatic wound infections (8,9), as well as decuibitus and diabetic skin lesions (10). Moreover, SSSI is an important risk factor for systemic infection.…”

“…Staphylococcus aureus causes a wide spectrum of clinically significant hospital-and community-acquired infections in human, including skin and soft-tissue infections and lifethreatening systemic infection (Daum, 2008;Pacheco et al, 2011;Karamatsu et al, 2012). The frequency of meticillinresistant S. aureus (MRSA) infections has now been declared a public-health imperative (Okesola, 2011;Dhand & Sakoulas, 2012;Stefani et al, 2012).…”
