Changes in Prealbumin, Nutrient Intake, and Systemic Inflammation in Elderly Recuperative Care Patients

Abstract: For older recuperative care patients, prealbumin and its change during hospitalization are positively associated with protein intake, but inflammation or changes in inflammation appear to exert a much more-powerful influence on prealbumin concentration. Given the potential confounding effects of inflammation, monitoring the change in prealbumin is not an adequate substitute for a more-detailed nutritional assessment in this population.

“…PA as a negative acute phase protein, 6-8 h after the stress state, can be a noticeable change. It has been confirmed serum PA levels decline in a variety of diseases, such as kidney injury, systemic inflammation, liver cirrhosis, brain trauma [13][14][15].…”

“…However, in these patients, the serum prealbumin provides prognostic value independent of the serum albumin and other established predictors of mortality in this population 3,7. In the older patients, repeated measurement of prealbumin is used to provide an assessment of the adequacy of nutrient intake over time 13. Several studies have advocated the use of serum prealbumin, as a better surrogate of nutritional status in HD patient population 9,14.…”

“…Measurement of hepatic proteins such as prealbumin has been repeatedly demonstrated to be useful for monitoring of malnutrition [214]. However, the presence of several kinds of inflammation is recognized to be the most important factor affecting the serum level of hepatic protein, due to capillary leakage or alteration of normal hepatic protein metabolism [7, 14–17].…”

“…However, the presence of several kinds of inflammation is recognized to be the most important factor affecting the serum level of hepatic protein, due to capillary leakage or alteration of normal hepatic protein metabolism [7, 14–17]. The inflammation-associated cytokines, especially IL-6 and TNF-α, were demonstrated to play a central role in the acute-phase response, which can include a decline in serum concentration of albumin and prealbumin and an increase in CRP [15].…”
