Purpose To determine the prevalence of pulmonary sarcoidosis in patients with uveitis from the University Referral Centre in Thailand and to report on their clinical characteristics. Methods We performed a retrospective review of results of radiological examinations of 209 consecutive new patients, diagnosed with uveitis. In patients with signs of pulmonary sarcoidosis, we reviewed clinical characteristics including age, gender, laterality and anatomical location of uveitis, and complications.Results From 209 chest X-ray (CXR) examinations, one patient exhibited radiological signs typical of stage 1 sarcoidosis. Chest CT of three patients with posterior multifocal chorioretinitis (PMC) revealed abnormalities suggesting the diagnosis of pulmonary sarcoidosis. All PMC patients were females older than 50 years; they had no pulmonary complaints and their CXRs were without abnormalities. Conclusion Ocular sarcoidosis is prevalent in Thailand and our findings suggest that possibly more patients with sarcoidosis would be identified if diagnosis was thought of and ancillary tests were performed.

Imaging has a prominent role in the assessment of sarcoidosis diagnosis and outcome, which are extremely variable. Chest radiography staging helps predict the probability of spontaneous remission, and stage IV is associated with higher mortality. However, the reproducibility of reading is poor and changes in radiography and lung function are inconsistently correlated, which may be problematic for the monitoring of disease and treatment response. Chest computed tomography (CT) makes a great diagnostic contribution in difficult cases. Bilateral hilar lymphadenopathy with peri-lymphatic micronodular pattern is highly specific for sarcoidosis. CT is important for the investigation of pulmonary complications, including aspergilloma and pulmonary hypertension. CT improves the yield of bronchoscopy for obtaining a positive endobronchial or transbronchial biopsy. CT findings may also discriminate between active inflammation and irreversible fibrosis, with occasional influence on therapeutic decisions. Three CT patterns of fibrotic sarcoidosis are identified, with different functional profiles: predominant bronchial distortion is associated with obstruction; honeycombing is associated with restriction and lower diffusing capacity of the lung for carbon monoxide; whereas functional impairment is relatively minor with linear pattern. The clinical impact of correlations between CT severity scores and functional impairment is uncertain, except for its utility elucidating the mechanisms of airflow limitation, which include bronchial distortion, peribronchovascular thickening, air-trapping and bronchial compression by lymphadenopathy.

To compare the clinical characteristics of uveitic sarcoidosis in African American and non-African American patients with biopsy-proven sarcoidosis and to determine which diagnostic test results were most often suggestive of sarcoidosis in patients who were ultimately diagnosed as having the disease. Method: Retrospective review of consecutive patients with biopsy-proven sarcoidosis evaluated by the uveitis service between 1989 and 2009. Results: A total of 63 patients with uveitic sarcoidosis were identified: 39 (62%) were African American (PϽ.001) and 43 (68%) were female. African American patients presented at an earlier age (PϽ.001) and were more likely to have granulomatous anterior segment inflammation (PϽ.001). The levels of serum markers angiotensin-converting enzyme and lysozyme were elevated in 40% and 42% of patients tested, respectively. The levels of at least 1 marker were elevated in 18 patients (58%). Imaging study results were reported as consistent with sarcoidosis in 25 patients (69%) who underwent chest radiography and in 19 patients (100%) who underwent computed tomography. Conclusions: In this series, African American patients were more likely to be diagnosed as having uveitic sarcoidosis and to present with uveitis if they were younger than 50 years. White patients were more likely to present when they were older than 50 years. A clinical picture that included granulomatous anterior segment inflammation was more common in African American patients. The use of serum markers (angiotensinconverting enzyme and lysozyme) positively identified more patients with biopsy-proven sarcoidosis when used in combination with appropriate chest imaging.
