Introduction A high degree of registration completeness is necessary in order to obtain unbiased and accurate register-based study results. We investigated the completeness of registration in the national Norwegian Arthroplasty Register (NAR).Material and methods Registration completeness for the years 1999-2002 was calculated as a percentage, with the number of joint replacements reported to the NAR as numerator and those reported to the Norwegian Patient Register (NPR) as denominator. While the NAR received information directly from the orthopedic surgeons on a voluntary basis, the NPR, which is mandatory, received information from the electronic administrative patient records of the hospitals.Results Registration completeness in the NAR was 97% (97% for primary operations; 101% for revisions). Completeness was 98% (97%; 106%) for hip replacements, and for knee replacements it was 99% (99%; 97%). Hip and knee replacements represented 95% of all operations. However, completeness was poorer for less common joint replacements and poorest for ankle implants (82%; 40%) and wrist implants (52%; 14%). In the NAR, completeness of registration of revisions involving only removal of one or more prosthetic parts was lower than for exchange revisions for all types of joint replacement. For hip implants, 76% of the removal revisions (80% of Girdlestone procedures) were reported, and for knee implants the figure was 62%. According to NPR statistics, removal procedures accounted for 9% of all revisions of hip and knee replacements.Interpretation In the NAR, registration completeness of hip and knee replacements was high both for

SAMMENDRAGOpplysninger om alle sykehusinnleggelser og polikliniske konsultasjoner ved norske offentlige og private sykehus som er knyttet til ordningen med innsatsstyrt finansiering innhentes og administreres av Norsk pasientregister (NPR). Ventelistedata administreres også av NPR. Registeret eies av Helsedepartementet som også finansierer driften, og drives av forskningsinstituttet SINTEF Helse hvor NPR er en egen organisasjonsenhet. Ved all kontakt med sykehus registreres data om pasientens alder, kjønn og hjemstedskommune, om institusjon og avdeling, om diagnose(r) og operasjonsprosedyre(r), om dato for inn-og utskriving og om dato for eventuelle operasjoner/prosedyrer. Registeret inneholder ikke navn eller fødselsnummer, kun et pasientnummer som følger pasienten ved én institusjon innenfor ett kalenderår. Hovedformålet med datainnsamlingen er administrativt og knyttet til ordningen med Innsatsstyrt finansiering av sykehusdrift. Data fra NPR benyttes i studier av tilgjengelighet, kvalitet og ressursutnyttelse innen helsetjenesteforskningen og i epidemiologisk forskning. Vi gjør i denne artikkelen rede for hvordan data fra NPR benyttes innen forskning. ENGLISH SUMMARYThe Norwegian Patient Registry covers nearly all inpatient and outpatient hospital care in Norway. The registry is owned and funded by the Government and run by SINTEF Health Research, a non-profit research organization. Data on the patient's age, sex and residence, hospital and department, diagnose(s) and surgical procedure(s), dates of admission and discharge and date of procedure are included in the registry. The unique personal identification number that each Norwegian citizen has is not included. The funding of Norwegian hospitals is partly based on the activity at each institution, and the Norwegian Patient Registry was established mainly for administrative purposes. Data from the registry are also used in research on availability, quality and utilization of health service resources, and in epidemiological research. This paper describes how data from the Norwegian Patient Registry is utilized for research purposes. INNLEDNINGI Norsk Pasientregister (NPR) lagres informasjon om alle sykehusinnleggelser og polikliniske konsultasjoner ved norske offentlige og private sykehus. Dataene innhentes på vegne av Helsedepartementet med hjemmel i §5 i Lov om spesialisthelsetjenesten. Hvert år registreres omtrent én million innleggelser og tre millioner polikliniske besøk. Dette registeret gir enkel tilgang til informasjon om alle sykdomstilfeller som har krevd behandling ved en somatisk eller psykiatrisk institusjon i Norge.

Laparoscopic and Open Cholecystectomy in Surgical Training

Background: Open cholecystectomy (OC) may still be necessary in surgical training to perform safe conversions of laparoscopic cholecystectomy (LC). Our aim was to study the outcome of LCs and OCs performed by surgical trainees. Methods: All consecutive cholecystectomies (1,581 LCs and 984 OCs) were retrospectively analyzed from 1995 until 2008. Operative complications were compared between the cholecystectomies performed by 20 trainees alone (n = 822), assisted operations (n = 754, trainees/specialist surgeons) and 9 specialists alone (n = 989). Results: Surgical trainees performed 787 (50%) LCs and 789 (80%) OCs either alone or assisted. The conversion rate of LC for trainees and specialist surgeons were 34 (7.0%) and 44 (5.5%), respectively. Complication rates and mortality were similar between the trainees and specialist surgeons. No bile duct injuries were associated with LCs or OCs performed by trainees alone. LCs were associated with 9 (0.57%) cases of bile leakage from cystic stump and 2 (0.13%) other bile duct injuries. Conclusion: Surgical trainees performed over half of our cholecystectomies with good results. The patient selection for LC versus OC was good, because no total transection of the common bile duct was observed in over 1,500 LC operations.
