Chronic Critical Limb Ischemia

Abstract: Though the data are still sparse, the high reported rates of limb preservation imply that peripheral revascularization techniques can play a major role in the treatment of chronic critical limb ischemia (CLI). Therefore, these techniques are recommended without exception by the current guidelines.

“…38 In another study, it has been indicated that postoperative wound healing is problematic in 15 to 25% of patients, the rate of bypass infections is 1 to 3% (especially higher in prosthetic grafts), and the rates of early perioperative and 1-year mortality are 1 to 2% and 10%, respectively. 39 Venous arterialization can be considered as last resort for limb salvage in CLI patients with no revascularization options. It is a unique surgical procedure in which the venous bed is used as an alternative conduit for perfusion of peripheral tissues.…”

“…На практике только у 37,3-58% больных выполняется адекватная реваскуляризация нижних конечностей [9]. В течение 1-го года положительный результат после операции сохраняется при реконструкциях выше щели коленного сустава в 49,1-72,3% наблюдений и при различных видах дистальных шунтирований в 10,2-39,1% случаев [10][11][12][13].…”

“…5,6 A conservative estimate of CLI patients in the United States is 250,000, of whom about one-third are poor or no-option patients. [7][8][9][10] A number of multiple gene-and cell-based therapies have been examined in PAD patients in general, and in no-option patients more specifically. 11 The application of angiogenic proteins and DNA encoding of such proteins have received considerable attention, but have failed to provide significant results.…”
