Background: Peritonsillar abscess is a collection of pus between the tonsillar fibrous capsule and the pharyngeal constrictor muscle. Unilateral peritonsillar abscess is a common complication of acute tonsillitis, whereas bilateral peritonsillar abscess is rare. The incidence of bilateral peritonsillar abscess remains unknown but is estimated to be 4.9% of all peritonsillar abscess cases. Case report: A 20-year-old man came to the emergency room with bilateral peritonsillar abscess and complained of shortness of breath for two weeks. Physical examination showed muffled voice, 82% oxygen saturation, and inspiratory stridor. The isthmus faucium was narrow, with approximately 30% remaining. Peritonsillar puncture revealed pus mixed with blood. The treatment for this patient included bilateral peritonsillar incision and drainage and intravenous antibiotics of levofloxacin and metronidazole. The patient came to the Ear, Nose, Throat, Head, and Neck (ENTHN) Outpatient Unit eight days after the procedure for a control checkup and was in good condition. Conclusion: We have reported a bilateral peritonsillar abscess with complications of upper airway obstruction, in which we performed incision and drainage. Prompt and appropriate management is needed to avoid unwanted morbidity and mortality.

Objective: Otitis media is a contagious inflammatory illness of the middle ear that might have a sudden onset and a complete resolution, or it can have a persistent presentation and long-term implications. Surgical treatment is available for all kinds of otitis media. Different types of operations include canal wall-up, canal wall-down, and their variations. The wall-up approach of the two canals with or without posterior tympanotomies, also known as tympanoplasty surgery, preserves the rear wall of the ear canal. Material and Methods: Hearing evaluation was conducted after canal wall-up tympanoplasty surgery at the outpatient clinic, Department of Otorhinolaryngology, Dr. Soetomo Hospital, Surabaya, Indonesia over a period of two years (2018-2019). Results: Two hundred and thirty-seven patients underwent from January 1, 2018 to December 31, 2019. However, only 74 (31.2%) of them had complete medical records after surgery. Fifty-nine ears (79.7%) experienced a better hearing threshold, six ears (8.1%) did not improve, and nine ears (12.2%) deteriorated. The hearing threshold improvement was of a magnitude of 10.9 dB; before surgery, it was 56.6 dB, and after surgery, it became 45.6 dB. Conclusion: A 10.9-dB improvement in hearing threshold mean was observed among patients undergoing canal wall-up tympanoplasty, whereas the bone gap improvement between pre- and post-tympanoplasty was 6.9 dB.

Background: Chronic suppurative otitis media (CSOM) is a persistent inflammatory disease of the middle ear and mastoid cavity caused by pathogenic infection. CSOM has a fairly high incidence in developing countries and is the main cause of acquired hearing loss in children. Tumor necrosis factor alpha (TNF-α) is a significant inflammatory mediator in CSOM. This study aimed to analyze TNF-α levels in ear discharge and blood serum in active phase CSOM caused by Gram-positive and Gram-negative bacteria.Materials and Methods: This research was an analytical observational study with a cross sectional design. Blood serum and ear discharge from CSOM patients were used in this study. Blood serum and ear discharge TNF-α levels were measured using enzyme-linked immunosorbent assay.Results: From 26 CSOM subjects, 13 subjects were infected with Gram-positive bacteria and the 13 others were infected with Gram-negative bacteria. The majority of the subjects were male (53.8%) with an age range from 36-45 years (42.3%). The most common species of bacteria was Pseudomonas aeruginosa. Blood serum and ear discharge TNF-α levels were higher in samples that contained Gram-positive bacteria.Conclusion: TNF-α levels in active phase CSOM caused by Gram-positive bacteria are higher than those which are caused by Gram-negative bacteria.Keywords: chronic suppurative otitis media, TNF-α, gram-positive, gram-negative
