Chronically Occluded Inferior Venae Cavae: Endovascular Treatment

Abstract: Endoluminal recanalization and stent placement in chronically occluded IVCs has a good intermediate-term outcome and should be considered in patients who have symptoms and who often do not have adequate alternative therapy.

“…Stents have the potential to improve patency in large veins by preventing recoil and allowing sufficient flow to maintain patency. The literature supports stenting and angioplasty in the IVC with improvement of symptoms and good patency (40). When chronic IVC obstruction cannot be recanalized via endovascular means, stenting to collateral drainage, possibly to the ascending lumbar vein, has been advocated with improvement in symptoms and good patency.…”

“…89 Stent insertion can improve the outcome of endoluminal interventions by preventing recoil therefore maintaining a large venous conduit to promote patency. 90 In patients with an unresectable underlying malignancy, treatments are commonly palliative. However, endoluminal treatment can relieve profound IVC obstructive/thrombotic symptoms, avoiding distressing and debilitating lower limb symptoms.…”

“…Most experience has been gained with Wallstents (Boston Scientific, Hemel Hempstead, Herts, UK) which are self expanding stents with a good radial strength. Studies have also shown the efficacy and durability of stents in the IVC (Ing et al, 2001;Razavi et al, 2000).…”
