Cigarette Smoking as a Triggering Factor of Hidradenitis suppurativa

Abstract: Background: Hidradenitis suppurativa is a chronic inflammatory skin disease involving the axillary, inguinal and anogenital regions and sometimes, in addition, the submammary or sacral areas. The etiology of this condition is unknown. Objective: A matched-pair case-control study was performed to evaluate the influence of smoking habits on the manifestation of this disease. “…The finding that 56% of the full analysis set were current cigarette smokers confirms previous reports of a relationship between smoking and HS 2, 22. This figure is striking when compared with the prevalence of smokers in the general Irish population, in which an estimated 19% of individuals aged ≥15 years smoke daily and a further 4% identify as occasional smokers 23…”

“…Wortsman and Jemec [41] suggested that magnetic resonance imaging or real-time spatial compound imaging may be helpful in establishing the true extent of disease before surgery. Lymph node involvement should only reflect secondary infection in severe HS [42]. We advise to associate sentinel lymph node dissection with surgical treatment of the carcinoma.…”

“…The belief that axillary disease is more common in women and anogenital disease more common in men has recently been refuted [3]. It seems to be associated with obesity and smoking [4,5]. The physiopathology of HS begins with follicular plugging, resulting in occlusion and dilatation of the pilosebaceous unit, with subsequent rupture.…”

“…In addition, fibroblast migration into lesional skin is inhibited (13,14). Smoking has been suspected to be a risk factor for skin diseases such as lupus erythematosus (15,16), palmoplantar pustulosis (17,18), psoriasis (19,20), and hidradentis suppurativa (21). In two of these diseases, hidradentis suppurativa and palmoplantar pustulosis, the data have indicated that the cessation of smoking may improve the disease (22,23).…”
