Cinacalcet and primary hyperparathyroidism: systematic review and meta regression

Abstract: Purpose: Primary hyperparathyroidism (PHPT) is common condition, affecting people of all ages and is mainly treated with parathyroidectomy. Cinacalcet has been widely used in secondary or tertiary hyperparathyroidism, but the use of cinacalcet in PHPT is less clear. Methods: Searches were conducted in Medline and Embase for cinacalcet use in PHPT from induction to 10th April 2020. Articles and conferences abstracts describing the use of cinacalcet for PHPT in prospective or retrospec… Show more

“…Data was extracted and analyzed using the metaprop and metareg functions on STATA (16.1 StataCorp LLC, Texas, TX, USA) [ 23 , 24 , 25 ]. The meta-analysis of proportions was conducted using a Freeman–Turkey double arcsine transformation for the stabilization of variance, and the Dersimonian and Laird random effects model [ 26 ] was used for the pooled analysis.…”

“…However, calcimimetic therapy has increasingly been reported to be useful in improving preoperative biochemical parameters in pHPT even before the COVID-19 pandemic [ 12 , 15 ]. The drop in efficacy of this therapy, and sometimes its biochemical normalization, was reported in the results of a randomized controlled study and a systemic review [ 16 , 17 ].…”

“…The majority of the studies were prospective and retrospective cohort studies. Four RCTs against placebo were also included, describing the use and efficacy of cinacalcet in a total of 722 PHPT patients [ 88 ]. Meta-regression reported a normalization rate of serum calcium of 90% (CI 0.82–0.96), with a mean reduction in calcium levels of 0.412 mmol/L (CI 0.343–0.481 mmol/L) from baseline.…”
