Cine magnetic resonance imaging vs high-resolution ultrasonography for detection of adhesions after laparoscopic and open incisional hernia repair: a matched pair pilot analysis

Abstract: Functional cine MR imaging represents a reliable noninvasive technique for detecting long-term adhesions after open and laparoscopic incisional hernia repair. The study results suggest that this approach has distinct advantages over high-resolution ultrasonography.

“…Furthermore, adhesions are reported in up to 57% of mesh area [12,13] and shrinkage was found to be almost half of the original surface area [13]. Comparable to this experimental results, within the clinical situation up to 70% of ePTFE meshes showed insufficient fixation, 80% had intra-abdominal adhesions, as well as a remarkably reduced mobility of the abdominal wall in 18% [14,15]. To further improve patient outcomes a new prosthesis was constructed.…”

“…As a result, other noninvasive diagnostic tools have been examined among these cine MRI [6,[20][21][22][23]. Compared with TAU, cine MRI not only enables one to visualize abdominal wall adhesions, but also adhesions between adjacent organs.…”

“…Ultrasonography may only detect adhesions with the anterior parietal peritoneum [23,24] while CT scanning does not seem to be of value in the detection of intra-abdominal adhesions. Cine MRI holds promise but information, especially regarding deeply located adhesions, is scarce [25]. An explanation for the accumulation of liposomes in early adhesive tissue is the locally increased vascular permeability of the peritoneal tissue after peritonitis.…”
