Circumferential Petrosectomy for Petrous Apicitis and Cranial Base Osteomyelitis

Abstract: The circumferential petrosectomy is a potential treatment option when medical treatment fails in patients with petrositis or cranial base osteomyelitis. It permits maximal temporal bone debridement while preserving hearing and facial nerve integrity in these life-threatening disease processes.

“…Fusobacterium necrophorum is an aggressive anaerobic organism that has been implicated other serious infections such as Lemierre's syndrome 18. These findings underscore the importance of aggressive, immediate management of advance otologic infections with surgery and intravenous antibiotics in collaboration with pediatric subspecialists in infectious disease 19…”

“…Advanced hyperbaric oxygen was shown to be effective in the treatment of advanced SBO (26). Surgical interventions may be necessary for the debridement of bone and inflammatory tissue, biopsy and culture (14). Staphylococcus aureus, serratia marcescens, escherichia coli and yeast reproduced in the purulent discharge taken from our patient's throat.…”

“…ICAs are often surrounded by inflammatory tissue; contrast involvement and wall thickening is seen. Stenosis may be seen in the petrous segment of the carotid due to petrositis (14) and in the extracranial segment due to retropharyngeal abscess (15). In addition, ICA stenosis can also be caused due to the mass effect or due to the spasms caused by the surrounding inflammatory tissue.…”

“…4). Narrowing of ICA lumen may also occur in the petrous segment associated with petrositis [13] or in the extracranial segments associated with retropharyngeal abscess [23]. Finally, ICA stenosis may result from simple mass effect or spasm associated with the surrounding inflammatory process.…”

“…Adjuvant hyperbaric oxygen therapy has proved effective in the treatment of advanced skul base osteomyelitis [4]. Surgery is reserved primarily for debridement of necrotic bone and inflammatory tissue, biopsy and culture [13]. The role of anticoagulation in the management of cerebrovascular complications in children remains debated because of its sporadic use and lack of controlled studies [14].…”
