Classification system for isolated arthritis of the scaphotrapeziotrapezoidal joint

Abstract: We have developed a classification system for osteoarthritis (OA) of the scaphotrapeziotrapezoidal (STT) joint that can be used on posteroanterior, lateral, and oblique wrist radiographs. It can be used to communicate the degree of severity of arthritis, and to study arthritis in the wrist further. Currently we know of no classification system for OA of the STT. We devised a classification system and made an initial study to assess the reproducibility of the system. It was compared with a classification system… Show more

“…The definition of OA was narrowing of the joint space (less than 1 mm or smaller than the other spaces in the same radiograph) with or without subchondral sclerosis, osteophytes, and cystlike defects. 3 The classification system used for the evaluation of STT joint OA is based on regular wrist radiographs. 3 This classification system does not include an evaluation of the trapezio-trapezoidal joint.…”

“…3 The classification system used for the evaluation of STT joint OA is based on regular wrist radiographs. 3 This classification system does not include an evaluation of the trapezio-trapezoidal joint. It defines the stage by the highest stage regardless of joint (scaphotrapezial/scaphotrapezoidal) and regardless of the view (PA, lateral, or oblique) of the wrist joint.…”

“…Primary osteoarthritis (OA) is an uncommon degenerative pathology of the wrist as compared with pathologies of other loading joints. The carpal scaphotrapeziotrapezoid (STT) is the second most affected joint by OA, [1][2][3] and it is present in 16% of wrist radiographs, [4][5][6] in 39% of cadaver dissections, 2 and in 83% of people older than 84 years of age. 1 The joint is located between the distal scaphoid and trapezius, and between the scaphoid and trapezoid bones.…”

“…arthritis. Prevalence of osteoarthritis (OA) of the wrist varies with age, 6 but has been found to be as high as 91% in patients older than 80 years, 7 and is generally estimated to affect between 11 8,9 and 15% of the population. 10 Treatment options for distal radius fractures include casting, with or without closed reduction, closed reduction with percutaneous pinning, external fixation alone, or in combination with pinning, and open reduction and internal fixation.…”
