Non-steroidal anti-inflammatory drugs (NSAIDs) are among the most frequently used classes of medications in the world, yet they induce an enteropathy that is associated with high morbidity and mortality. A major limitation to better understanding the pathophysiology and diagnosis of this enteropathy is the difficulty of obtaining information about the primary site of injury, namely the distal small intestine. We investigated the utility of using mRNA from exfoliated cells in stool as a means to surveil the distal small intestine in a murine model of NSAID enteropathy. Specifically, we performed RNA-Seq on exfoliated cells found in feces and compared these data to RNA-Seq from both the small intestinal mucosa and colonic mucosa of healthy control mice or those exhibiting NSAID-induced enteropathy. Global gene expression analysis, data intersection, pathway analysis, and computational approaches including linear discriminant analysis (LDA) and sparse canonical correlation analysis (CCA) were used to assess the inter-relatedness of tissue (invasive) and stool (noninvasive) datasets. These analyses revealed that the exfoliated cell transcriptome closely mirrored the transcriptome of the small intestinal mucosa. Thus, the exfoliome may serve as a non-invasive means of detecting and monitoring NSAID enteropathy (and possibly other gastrointestinal mucosal inflammatory diseases).

Introduction: Diclofenac (DFC) has been widely utilized as anti-inflammatory and analgesic medication. Naringin is a natural flavanone glycoside that is found in citrus fruits. Quercetin is a natural flavonoid found in vegetables and fruits. Aim: evaluation of the roles of naringin and quercetin in protection and recovery of rats' stomach ulcers caused by DFC. Material and methods: Forty male albino adult rats were divided into 4 identical groups: Control group, (DFC) group: rats were given 10 mg/kg b.w. /day DFC by oral gavage for 4 weeks, (DFC + Naringin) group: rats were given100 mg/kg/body weight naringin daily along with previous dose of DFC dose for 4 weeks and (DFC + Quercetin) group: rats were given 50 mg/kg body weight quercetin daily along with previous dose of DFC. The stomach tissues were examined grossly and processed for microscopic examination. Results: DFC group revealed ulceration of mucosa resulting in detachment of fundic mucosa, inflammatory infiltration, and wide lumen of fundic glands. There was a thin, sporadic PAS-AB mucous coating over the surface epithelium. It showed extensive iNOS immunoreaction in the cytoplasm of gastric epithelial cell and strong PCNA immunoexpression in the cells lining the fundic glands. Both naringin and quercetin exhibited a protective effect by prevention of histopathological changes caused by DFC on gastric mucosa. There is a persistent, thick mucus coating covering the surface epithelium, weak iNOS immunoreaction and moderate PCNA. Conclusion:The intake of naringin during taking diclofenac protects the stomach mucosa but quercetin has more protection than naringin.

The incidence of inflammatory bowel disease with its two main manifestations, colitis ulcerosa and Crohn’s disease, is rising globally year after year. There is still a tremendous need to study the underlying pathomechanisms and a well-established tool in order to better understand the disease are colitis models in rodents. Since the concept of the 3Rs was proposed by Russell and Burch, this would include pain medication in animal models of intestinal inflammation as a reduction of suffering. This review argues against pain medication because the administration of pain medication in its current form has an impact on the inflammatory process and the immune response, thus falsifying the results and the reproducibility and therefore leading to misconceptions.
