Clinical Parameters and Tools for Home-Based Assessment of Parkinson’s Disease: Results from a Delphi study

Abstract: Abstract. Background: Parkinson's disease (PD) is a neurodegenerative disorder with fluctuating symptoms. To aid the development of a system to evaluate people with PD (PwP) at home (SENSE-PARK system) there was a need to define parameters and tools to be applied in the assessment of 6 domains: gait, bradykinesia/hypokinesia, tremor, sleep, balance and cognition. Objective: To identify relevant parameters and assessment tools of the 6 domains, from the perspective of PwP, caregivers and movement disorders speci… Show more

“…Indeed, changes in QoL has been chosen as the principal variable in some studies with PD patients selected for receiving a second line therapy [20,21] and the use of PDQ-39 in routine care for PD has been suggested [22]. However, although some criteria have been proposed with the aim to define advanced PD [23][24][25], in many cases the criteria are not useful in daily clinical practice because their application is time-consuming and also because the concept of PD is too broad [26]. erefore, a quick screening tool that identifies which PD patients are worse and need optimized treatment, including possibly a second line therapy, would be ideal.…”

“…The Delphi methodology was selected as the approach for this study given the large body of published evidence of the use of Delphi methods in clinical practice and health research to explore topics in healthcare that have not been previously examined [25][26][27] . The…”

“…An ESM app, the Psymate, was downloaded and installed on the smartphone of the participants. We developed a specific ESM questionnaire, based on literature discussing relevant symptoms and items for monitoring PD at home 28,29 and based on patient and clinician interviews about relevant symptoms and items for PD monitoring at home. During the measurement period, participants were asked to complete a morning (five questions) and evening questionnaire (eight questions) which were identical on all days and which were available in the morning and evening (Supplementary Fig.…”
