Background: As children with diarrhoea are often prescribed antibiotics, it was decided to study the causative organisms. Rotavirus is a major cause of diarrhoea and diarrhoea related mortality in most parts of India. Rotavirus vaccine is available but expensive. According to the World Health Organisation (WHO) the vaccine should be used in areas where diarrhoea related under five mortality is around 10%. The situation in north Karnataka is not known. Objectives: To find proportions of rotavirus and nonrotavirus diarrhoea in children of north Karnataka and study the key clinical features of rotavirus diarrhoea. Study design and settings: Prospective observational study conducted in patients from the paediatric ward and outpatient department in a tertiary care hospital. Method: Children 1 to 36 months old, presenting with acute diarrhoea from July 2014 to May 2016 and fulfilling selection criteria were included. Demographic and clinical details were noted. Stools were collected within 24 hours and subjected to routine and microscopic examination, test for presence of reducing substance, bacterial culture and Rapid ELISA test for rotavirus using antigen detection micro-well ELISA kit by Premere Rotaclone. Results: A total of 168 children was recruited. Children positive for rotavirus ELISA were labelled as having rotavirus diarrhoea and comprised 52 (31%) of total, including 17 with mixed infection and 15% were bacterial culture positive. Most common age group affected was 7 to 18 months (79%). Male female ratio was same in rotavirus and non-rotavirus diarrhoea.

Since its discovery 40 years ago, rotavirus (RV) is considered to be a major cause of infant and childhood morbidity and mortality particularly in developing countries. Nearly every child in the world under 5 years of age is at the risk of RV infection. It is estimated that 90% of RV-associated mortalities occur in developing countries of Africa and Asia. Two live oral vaccines, RotaTeq (RV5, Merck) and Rotarix (RV1, GlaxoSmithKline) have been successfully deployed to scale down the disease burden in Europe and America, but they are less effective in Africa and Asia. In April 2009, the World Health Organization recommended the inclusion of RV vaccination in national immunization programs of all countries with great emphasis in developing countries. To date, 86 countries have included RV vaccines into their national immunization programs including 41 Global Alliance for Vaccines and Immunization eligible countries. The predominant RV genotypes circulating all over the world are G1P[8], G2P[4], G3P[8], G4P[8], and G9P[8], while G12[P6] and G12[P8] are emerging genotypes. On account of the segmented genome, RV shows an enormous genetic diversity that leads to the evolution of new genotypes that can influence the efficacy of current vaccines. The current need is for a global RV surveillance program to monitor the prevalence and antigenic variability of new genotypes to formulate future vaccine development planning. In this review, we will summarize the previous and recent insights into RV structure, classification, and epidemiology and current status of RV vaccination around the globe and will also cover the status of RV research and vaccine policy in Pakistan.

Aims: Rotavirus (RV) is the most important etiological agent of diarrhea in children with high morbidity and mortality, mainly in developing countries. A big number of children from one to five year old have been infected by RV, with at least one type of virus. Data show that human body can be infected repeatedly by different strains of RV. It is predicted that there exist 16 types of G genotypes of RV. This study was aimed to identify the G genotypes of RV in children with diarrhea in Pekanbaru, Indonesia. Methodology and results: This research is a cross-sectional study involving children aged zero to sixty months from January to July 2015. The stool samples were collected from diarrhea patients and the identification of RV was done by using the rapid test. The RNA isolation was performed on positive isolates. The identification of the G genotypes was performed by using a semi nested reverse transcription PCR method. The study involved 71 children with diarrhea. VP7based RV detection showed 47 positive samples (66.1%). The predominant G type of the positive results are G1, G9 and G3 namely 36.2%, 25.5% and 12.8%. The average age of the subjects was 15.3±2.3 months. The majority of the subject were females (53.6%). Most of the diarrhea feces presented in this study were neither haemorrhagic nor liquefied. The subjects in this study are also presented with co-symptoms such as fever (100.0%), gastrointestinal disorder (94.6%) and dypsnea (16.1%). Conclusion, significance and impact study: Based on this study, we conclude that the predominant G types is G1. This study was performed to identify the predominant G genotype of RV and designed the antigen-antibody based in diagnose RV.
