Primary gastric adenosquamous carcinoma (ASC) is a rare tumor, and gastric carcinoma with lymphoid stroma (GCLS) is a distinct and relatively rare tumor, which is characterized histopathologically by prominent lymphoid infiltration and is commonly associated with Epstein-Barr virus (EBV) infection. The association between ASC and GCLS has been poorly understood, and only two cases of concomitant occurrence of ASC and GCLS have been reported previously. In this report, we describe the first documented case of simultaneous gastric ASC and GCLS. A 58-year-old Japanese male complained of loss of appetite. Gastric endoscopic examination revealed a type 3 tumor in the antrum. Distal gastrectomy with lymph node dissection was performed after neoadjuvant chemotherapy. Macroscopic examination of the resected specimen revealed two discontinuous lesions in the antrum. The first lesion, an ASC, was detected preoperatively by endoscopic examination. This type 3 tumor was composed of a mixture of squamous cell carcinoma (~75% of the tumor) and poorly-to-moderately differentiated adenocarcinoma (~25%). No EBV-encoded RNA (EBER)-positive neoplastic cells were noted. The tumor had invaded into the serosa. The second lesion was a GCLS, which was not preoperatively detected by endoscopic examination. The neoplastic cells of the GCLS were EBER-positive. The tumor had invaded into the muscularis propria. Accordingly, a diagnosis of simultaneous gastric ASC (pT4a) and GCLS (pT2) was made. The pathogenesis of gastric ASC has been under debate. Only limited cases of ASC associated with EBV have been reported. Notably, EBV has not been associated with the development of an ASC component in the previously reported concomitant cases of ASC and GCLS, as well as in the present case study. Therefore, additional studies are required to clarify the pathogenesis of gastric ASC, including its association with EBV.

Adenosquamous Carcinoma of the Breast: A Population-Based Study Using the SEER Database

Background: The present study is aimed at summarizing the clinicopathological characteristics, prognosis, and management of breast adenosquamous carcinoma (ASC). Methods: A population-based study was performed using retrospectively extracted data from the Surveillance, Epidemiology and End Results database for breast cancer patients with histological diagnoses of ASC, infiltrating duct carcinoma (IDC) and squamous cell carcinoma (SCC) from 2004 to 2016. End-points were overall survival (OS) and breast cancer-specific mortality (BCSM). Propensity Score Matching (PSM) was employed to minimize selection bias of baseline characteristics. Univariable and multivariable analyses were used for identifying valuable prognostic factors. Results: ASC presented similar tumor size but low histological grade and less lymph node metastasis compared to IDC. ASC expressed less positive rate of hormone receptors and barely HER2, which was similar with SCC (estrogen receptor (ER): ASC 27.74% and SCC 21.53%, progesterone receptor (PR): ASC 18.06% and SCC 12.85%, HER2: ASC 4.44% and SCC 7.53%). ASC patients underwent the same treatment as IDC (chemotherapy 36.99% vs. 41.86%, BCS 50.58% vs 52.83%, P >0.05), only with less radiotherapy (39.88% vs. 48.34%, P<0.05). Median follow-up data of 78 months showed that the prognosis of IDC patients was better than that of ASC patients (all P <0.05 for BCSM and OS). After adjustment for clinicopathological and therapeutic factors in Cox proportional hazards models, ASC was no longer an independent poor prognosis factor. In matched groups, no significant difference in BCSM nor OS was observed between ASC and IDC groups. In HR-negative patients, the prognosis of ASC was similar with that of IDC, and both were superior to SCC. In HR-positive patients, the five-year survival rate of ASC was only about 60%, which was far less than that in ASC of HR-negative, the poor prognosis of ASC was closer to that of SCC. Multivariate analysis showed that older age (age≥60) and advanced AJCC stage (III and IV) were independent factors of poor prognosis in ASC, breast-conserving surgery was also ideally suited for ASC. Conclusions: ASC has unique clinicopathological characteristics and prognosis. To improve the clinical and biological understanding of ASC can make breast cancer patients get more individualized treatment.

Objective: Gastric cancer consists of many histological subtypes. Prognostic value of histological types in gastric cancer has not been very well defined. In this study, we aimed to investigate the relationship between different histological types and clinicopathologic features and prognosis in gastric cancer. Method: Patients whose pathological diagnosis was adenosquamous carcinoma, hepatoid adenocarcinoma, lymphoepithelioma-like carcinoma, and papillary adenocarcinoma, among the 1060 patients who underwent gastrectomy for gastric cancer between 2010-2019, were included in the study. Demographic features, clinicopathological features, oncological follow-up results and survival of the patients were analyzed. Results: Group 1 (adenosquamous carcinoma) consisted of 3, Group 2 (hepatoid adenocarcinoma) of 3, Group 3 (lymphoepithelioma-like carcinoma) of 3, and Group 4 (papillary adenocarcinoma) consisted of 4 patients. The mean age of the patients included in the study was 63.3+11.76 (41-81) years. Tumors were more commonly located in the corpus in hepatoid adenocarcinoma and lymphoma-like carcinoma (67%) and in the antrum (75%) in papillary adenocarcinoma. Eight patients underwent total gastrectomy, four patients subtotal gastrectomy, and a patient with a tumor located at the gastroesophageal junction underwent proximal gastrectomy. Average tumor size (cm) was 5.11+2.23 (1.2-8) cm. Local recurrence occurred in two patients with adenosquamous carcinoma, two with hepatoid adenocarcinoma, and one with papillary adenocarcinoma. Two patients with adenosquamous carcinoma developed systemic metastasis (lung, liver), two patients with hepatoid adenocarcinoma developed peritoneal carcinomatosis, and a patient with papillary adenocarcinoma developed surrenal metastasis. Average survival was the shortest in hepatoid adenocarcinoma (17.50 months), and the longest in papillary adenocarcinoma (63 months). There was no statistical difference in survival between the groups (p: 0.445). Conclusion: Rare histological types of the stomach differed in terms of their locations and prognoses. Among the rare histological types, hepatoid adenocarcinoma exhibited the most aggressive biological behavior, while patients with papillary adenocarcinoma had longer survival times.
